摘要
目的研究栓道式附着体在下颌倾斜基牙修复中的临床应用效果。方法对19例下颌第一磨牙缺失导致基牙和（或）邻牙严重倾斜，常规备牙无法取得共同就位道的病例，采用分段式设计，利用栓道式附着体相连接，制作完成19件烤瓷固定桥。观察修复体的吻合性、稳固性、咀嚼功能、对牙周组织的影响。结果经过1．6年的观察，有1例在戴牙3年后出现附着体结合部轻微松动。其余病例咀嚼功能良好，未出现冠或附着体的松动，无明显牙龈红肿及基牙松动，无食物嵌塞，无牙髓炎及根尖周炎症。结论分段式烤瓷固定桥在下颌倾斜基牙修复中的临床应用效果良好。
Objective To investigate the clinical result of sectional PFM fixed bridges on tilted submandibular abutments. Methods We made the sectional design, utilized the key-key way attachments to connect the two parts of PFM fixed bridges in such cases in which the first molar in submandible was lost,abutments and/or adjacent teeth were tilted, or the common path of insertion couldn＇t be found after common teeth preparations. 19 bridges in 19 cases were made. We observed the fitness, stabilization, masticatory function of restorations and their influence on periodontal tissue. Results Looseness between the two parts of key-key way attachment was found in one restoration three years after it was cemented . In other cases, masticatory function was good. Looseness of crown and attachment, gingivitis, abutment looseness, food impaction, pulpitis or periapical periodontitis were not found. Conclusion The sectional PFM fixed bridge is a good restoration for tilted submandibular abutments.
