摘要
目的探讨动脉瘤性SAH患者脑脊液中淀粉样β蛋白(Aβ)、载脂蛋白E(ApoE)及S100B蛋白水平的变化及其与病情的关系。方法以酶联免疫吸附试验(ELISA)法检测25例SAH患者(SAH组)脑脊液中Aβ、ApoE及S100B的含量,另取15例无神经系统疾病患者的正常脑脊液作为对照组。测定脑脊液S100B蛋白含量作为判定脑损伤的指标,以GOS作为判断预后的指标。结果SAH组脑脊液Aβ和ApoE含量分别为(5.4±1.2)、(0.462±0.007)μg/L,比对照组(8.4±1.6)和(0.849±0.114)μg/L显著下降(P=0.0083和0.0076);S100B含量为(18.6±7.3)pg/L,比对照组(6.6±1.0)pg/L显著增高(P=0.0069);脑脊液中Aβ、ApoE、S100B含量在GOSⅠ~Ⅲ级患者分别为(3.8±1.0)、(0.325±0.022)μg/L及(26±10)pg/L,在GOSⅣ~Ⅴ级患者中分别为(6.0±1.0)μg/L、(0.579±0.077)μg/L及(15±7)pg/L,GOSⅠ~Ⅲ级患者与Ⅳ~Ⅴ级患者相比,脑脊液中Aβ和ApoE含量明显降低(P=0.0086和0.0065),S100B含量则较高(P=0.0044)。结论SAH患者脑脊液Aβ和ApoE水平下降,可能与SAH患者脑损伤有关。检测两者在脑脊液中的含量,可作为判断预后的一个指标。
Objective To investigate the temporal alterations of cerebrospinal fluid (CSF) amyloid betaprotein, apolipoprotein E (ApoE) and S100B in patients with aneurysmal subarachnoid hemorrhage (SAH) and its relations with the disease. Methods The levels of CSF amyloid beta - protein and apolipoprotein E in 25 patients with SAH were measured by enzyme linked immunosorbent (ELISA). The normal CSF of 15 patients without nervous system diseases was used as controls. The measurement of the levels of CSF S100B was used as an indicator for identifying brain injury. Glasgow outcome scale (GOS) was used as an indicator for predicting prognosis. Results The levels of CSF amyloid beta - protein and ApoE in patients with SAH were 5.4 ± 1.2 and 0. 462 ±0. 007 μg/L respectively, and they decreased significantly as compared with the controls ( 8. 4 ± 1.6 and 0. 849 ± 0. 114 μg/L) ( P = 0. 0083 and 0. 0076) ; the levels of S100B was 18.6 ± 7.3 pg/L, and they increased significantly as compared with the controls ( 6. 6 ± 1.0 pg/L) ( P = 0. 0069 ). The levels of CSF amyloid beta - protein, ApoE and S100B in COS Ⅰ - Ⅲ were 3. 8 ± 1.0 μg/L, 0. 325 ± 0. 022 μg/L, and 26 ± 10 pg/L respectively, and in GOS Ⅳ and Ⅴ were 5.9 ± 1.2 μg/L, 0. 579 ±0. 077 μg/L, and 15 ±7 pg/L, respectively. As compared between GOS Ⅰ - Ⅲ and COS Ⅳ/Ⅴ, the levels of CSF amyloid beta-protein and ApoE were significantly lower (P = 0. 0086, 0. 0065 ) , but the levels of S100B were higher ( P = 0. 0044). Conclusion The levels of CSF amyloid betaprotein and ApoE are all decreased in patients with SAH, that may be associated with brain injury in patients with SAH. The detection of their levels in CSF can be used as an indicator for predicting prognosis of the patients.
