摘要
目的了解女性系统性红斑狼疮(SLE)患者血清性激素及相关细胞因子水平,并探讨其与患者病情发生、发展的关系。方法用电化学发光法检测雌二醇(E2)、睾酮(T)、孕酮(Prog)、泌乳素(PRL);放射免疫法检测雌三醇(E3)、白介素-2(IL-2)、白介素-6(IL-6)、白介素-8(IL-8),ELlSA法检测白介素-4(IL-4)和γ干扰素(IFN-γ)。结果女性SLE患者E2[(53.44±32.10)μg/L]、E3[(4.98±1.60)μg/L3、Prog((1.78±0.97)μg/L]、PRL[(18.18±13.74)μg/L]水平均明显高于健康对照组(P〈0.05),患者活动期的E2[(67.63土33.55)μg/L]、μg/L]、Prog[(1.82土0.79)μg/L]和PRL[(23.48±16.04)μg/L]水平明显高于稳定期和健康对照组(P〈0.05)。患者IL-4水平活动期[(21.35±3.60)ng/L]和稳定期[(19.93±3.93)ng/L]均明显比健康对照组高(P%0.05),而IFN-γ水平活动期[(12.67±0.74)ng/L]和稳定期[(13.02±1.96)ng/L]却明显比健康对照组低(P〈0.05)。结论E2、Prog、PRL水平与SLE病情活动密切相关。IL-4水平明显高于健康对照组,IFN-γ水平明显低于健康对照组(P〈0.05),支持SLE患者体内存在Th1细胞向Th2细胞转化,表现为以Th2细胞为主的免疫应答。
Objective To investigate serum levels of sex hormones and related cytokine levels in female patients with systemic lupus erythematosus (SLE), and to explore the relationship among the hormones as well as related cytokines and genesis and development of SLE. Methods Serum levels of estradiol (E2), testosterone (T), progesterone (Prog) and prolactin (PRL) were determined by electrochemiluminescence immunoassay; IL-4 and interferon gamma (IFN-γ).were measured by ELISA; and while estriol (E3) ,IL-2,IL-6 and IL-8 were determined by radioimmunoassay. Results The serum level of E2, E3, Prog and PRL in SLE group was (53.44±32.10), (4.98±1.60) ,(1.78±0.97)μg/ L and(18.18±13.74)μg/L, much higher than those in the control group (P〈0.05); while the level of IL-4 in active stage group E(21. 35±3. 60) ng/L] and stable phase group [(19. 93±3. 93) ng/L)] was significantly higher than that in active stage group [(12.67±0.74) ng/L] and stable stage group [(13. 02±1. 96) ng/L] was significantly lower than those in the control group (P〈0. 05). Conclusion The serum levels of sex hormones such as E2, Prog and PRL are closely related to SLE activity. Variations of IL-4 and IFN-γ in SLE indicate that the shift of Thl to Th2 as well as Th2 cell dominated immune response present in patients with SLE.
