摘要
目的评价CVTP方案治疗恶性淋巴瘤的临床疗效和毒副作用.方法38例恶性淋巴瘤患者,男30例,女8例,年龄29～75岁,其中小于60岁组27例,大于60岁组11例.根据治疗经过分为初治组18例,复发和难治组20例,非霍奇金淋巴瘤32例,霍奇金淋巴瘤6例,治疗采用环磷酰胺(CTX)0.6～0.8g/次,第1天用,西艾克(VDS)3～4mg/次,第1天用,吡哺阿霉素(吡柔吡星,THP)20 mg/次,第1～3天用,强的松(Pred)1 mg/kg,第1～7天用,根据血象,肝功情况,每14～21d重复治疗.结果总有效率84%,小于60岁组有效率88%,大于60岁组为89%,两组间疗效无显著性差异.初治组有效率100%,复治和难治组为50%。结论CVTP方联合化疗初治组疗效优于复治和难治组.毒副作用为轻度骨髓抑制。
Objective To evaluate the clinical therapeutic effect of CVTP regimen on patients with malignant lymphoma. Methods 38 patients with malignant lymphoma （30 males, 8 females） were evaluated. Of these 38 patients, there were 32 non -Hodgkin＇ s lymphoma and 6 Hodgkin＇ s lymphoma, and their age ranged from 29 - 75 （ 11 were over 60 years old）. All patients were treated with Cyclophosphamide （CTX） 600 - 800 mg and Vindesine （VDS） 3 - 4 mg on day 1, THP 20 mg on day 1, 2, 3, Pred 1 mg/kg on day 1 - 7. Patients were monitored with blood routine and liver function. All patients were evaluated after 2 cycles （ every cycle lasted 14 -21 days）. The effective rate and the adverse effects in different age groups （ 〈60 years vs≥60 years） and different therapeutic passing groups （previously untreated vs. relapsed/refractory） were compared. Results Total effective rate was 84% , the response rate was 88% in patients less than 60 years old and 89% in patients over 60 years old. The inter- group comparison showed no significant difference. In previously untreated patients and relapsed/refractory patients, the effective rate was 100%, and 50%, respectively. Conclusions The clinical therapeutic effect of CVTP regimen in previously untreated patients was better than that in relapsed/ refractory patients. The main adverse effect was mild myelosuppression.
