比较Ni-Ti螺旋弹簧和橡皮链施力后牙齿向远中移动速率及龈沟液中细胞间黏附分子1变化 被引量:6

摘要 目的:机械张应力可通过抑制成骨细胞细胞间黏附分子1mRNA的表达,限制破骨细胞的分化成熟,而不同正畸力可能起不同的作用。实验观察Ni-Ti螺旋弹簧和橡皮链施力后,尖牙远中移动速率、龈沟液中细胞间黏附分子1水平,探讨成骨细胞细胞间黏附分子1水平变化和不同持续牵张力的正畸效果和生物学效应。方法:选择2003-09/2006-08在桂林医学院附属医院进行口腔正畸治疗,双侧对称拔除四颗第一双尖牙的恒牙牙合患者36例。采用0.022英寸系统的OPAK直丝弓托槽使牙齿排齐整平后,采用不锈钢方丝进行尖牙远中移动,左右两侧按随机法原则分别用Ni-Ti螺旋弹簧(直径0.012英寸,长10mm)和橡皮链施力。用初始力150g牵引尖牙向远中,21d时测定牙齿移动距离,分别在加力前、加力后21d和6个月复诊时采集龈沟液量,利用细胞间黏附分子1试剂盒,用ELISA法对上清液进行龈沟液中细胞间黏附分子1水平的检测。结果:36例患者进入结果分析。①尖牙移动距离:施力21d后,螺旋弹簧侧上颌尖牙和下颌尖牙移动距离均大于橡皮链侧(P<0.01)。②龈沟液中细胞间黏附分子1质量浓度:螺旋弹簧和橡皮链两侧在加力前比较差异无显著性意义(P>0.05);加力后,两侧龈沟液中细胞间黏附分子1质量浓度均降低,加力21d和6个月时与加力前比较差异均有非常显著性意义(P<0.01);加力21d和6个月时螺旋弹簧施力侧龈沟液中细胞间黏附分子1质量浓度均低于橡皮链施力侧,差异有非常显著性意义(P<0.01)。结论:①螺旋弹簧拉尖牙向远中移动速率较快,产生的生物学效应较强而持久。②不同牵张力对细胞间黏附分子1水平变化起不同作用,产生的生物学效应也不同,矫治力在牙周组织的分布,即应力的大小和分布对正畸牙的牙周组织反应具有决定作用。 AIM: Mechanical tensile stress can inhibit the expression of osteoblast intracellular adhesion molecule 1 (ICAM-1) mRNA and restrict the differentiation of osteoclasts. Different orthodontic forces induce different effects. This study is designed to study the rate of distal and middle movement of canines and the ICAM-1 activity in gingival crevicular fluid (GCF) after retracted by Ni-Ti coil spring and elastic chain, in order to probe into the orthodontic and biological effects of the different permanent strains. METHODS: A total of 36 patients from the Affiliated Hospital of Guilin Medical College between September 2003 and August 2006 were treated with OPAK system of 0.022 inch straight wire appliance to pull out four permanent teeth of first bicuspid in symmetrical order. Then they were pushed on each side to move distally by Ni-Ti coil spring (0.012 inch diameter and 10 mm length) and elastic chain at random with 150 g original force. And 21 days later, the moving distances of canines were determined, GCF was collected before loading, 21 days and 6 months after exerting force respectively, then ICAM-1 level in GCF was tested with the method of enzyme-linked immunosorbent assay. RESULTS: All the 36 patients were involved in the result analysis. ①After 21 days' retraction, the moving distances of canines in Ni-Ti coil spring side were longer than those in elastic chain side (P 〈 0.01). ②No difference was found between Ni-Ti coil spring side and elastic chain side before exerting force (P 〉 0.05). Then GCF-ICAM-1 activity dropped as the time went by, and there were extremely significant differences in 21 days and 6 months after exerting force compared with that before exerting force; The ICAM-1 activity in coil spring side, compared with that in elastic chain side, was extremely significantly lower (P 〈 0.01 ). CONCLUSION: ①Compared with elastic chain, there is a higher distal and middle moving rate and more permanent biological effects in coil spring. ② Different
