免疫细胞化学法检测尿足细胞的临床应用 被引量:7

摘要 目的:免疫荧光细胞化学和免疫酶细胞化学两种方法检测尿足细胞的比较,以促其临床推广应用。方法:尿标本均为我科住院患者的肾活检日新鲜晨尿,采用抗人足细胞标记蛋白Podocalxin(PCX)单克隆抗体进行尿沉渣免疫荧光细胞化学和免疫酶细胞化学染色。14例患者均经肾活检,肾组织由光镜、免疫荧光和电镜检查做出病理诊断。结果:(1)14份尿标本应用免疫荧光细胞化学和免疫酶细胞化学方法均可检测到足细胞。(2)同一份尿沉渣采用免疫荧光细胞化学法检测足细胞均数为(16.7±15.1)/HP,而免疫酶细胞化学法检测足细胞均数为(26.1±24.3)/20HP,两组比较有统计学意义(P<0.05);但两种方法所测足细胞数之间有很好的相关性(r=0.969)。结论:免疫荧光细胞化学和免疫酶细胞化学两种方法检测尿足细胞,前者较简便、镜下易辨认,后者设备简单、标本可保存。 Objective:To apply of immunocytochemistry method for detection of urinary podocytes. Methods:Urine samples from 14 patients with glomerulonepheritis were collected at Peking University First Hospital. They all received percutaneous renal biopsy and had been made pathologic diagnosis by light, immunofluorescence and electron microscope. Fresh urine samples were obtained in the morning of the day when they received renal biopsy before treatment. Urine samples we collected from three patients continuously for three days, which were obtained as control. Urine samples were stained with monoclonal mouse anti- human Podocalyxin (PCX) antibody by Immunoenzymatic celluar chemistry and indirect Immunofluoreseence celluar chemistry cells, respectively. Results: (1)All the 14 urine samples had positive staining of PCX either with Immunoenzymatic celluar chemistry or Immunofluorescence celluar chemistry. (2)More positive ceils could be detected by Immunoenzymatic celluar chemistry than by Immunofluorescence celluar chemistry[ (26.1±24.3)/HP vs ( 16.7 ± 15.1 )/HP] in the same urine .sample, but the amount of positive cells in each sample had good correlation between two methods ( r = 0. 969). Conclusion: Immunoenzymatic celluar chemistry can be used to detect urinary podocytes and is more suitable for local hospital than Immunofluorescence celluar chemistry because of the convenience and cheap equipment needed, and the stained samples can be kept for long time.
