摘要
目的:研究G raves眼病(GO)患者血清中TNF-α、IL-6的表达水平及其在免疫反应中的作用,以探讨细胞因子对GO的影响。方法:对45例G raves病未经治疗的非突眼患者、36例G raves眼病(GO)患者及36例正常者(对照组)检测了血清中TNF-α、IL-6的表达水平和甲状腺激素及相关抗体水平。TNF-α、IL-6测定及抗甲状腺球蛋白抗体(TgAb)、抗甲状腺微粒体抗体(TmAb)测定采用放射免疫分析法(R IA);血清总T3(TT3)、总T4(TT4)、促甲状腺激素(TSH)测定采用化学发光免疫分析法。结果:与正常对照组比较,各组TNF-α、IL-6显著增高(P<0.01)。GO、GD非突眼组中TNF-α和TNF-α/IL-6比值与TgAb、TmAb存在正相关(P<0.01)。结论:TNF-α、IL-6共同参与G raves病的发生。GD非突眼患者以IL-6升高为主,提示Th2所介导的体液免疫占优势;GO患者TNF-α、IL-6均增高,TNF-α/IL-6比值与对照组无统计学差异,提示GO的免疫反应呈Th0型,细胞免疫和体液免疫共同参与眼病的发生。
Objective:To investigate the serum level of tumor necrosis factor alpha(TNF-alpha) and interleukin-6(IL-6) in patients with graves'ophthalmopathy(GO) as well as its immunological mechanisms.Methods:TNF-alpha,IL-6,thyroid hormone and correlative antibody of thyroid were determined in 45 patients with graves'disease(GD) without ophthalmopathy,36 patients with graves'ophthalmopathy(GO) and 36 control cases.The level of TNF-alpha,IL-6,TgAb and TmAb were measured by radioimmunoassay(RIA).The levels of total T3(TT3),total T4(TT4) and thyrotropin hormone(TSH) were examined with chemiluminescent immunoassay.Results:Compared with the control,the level of TNF-alpha and IL-6 were significantly elevated in GO(P〈0.01).The level of TNF-alpha and TNF-alpha/IL-6 ratio were positively correlated with TgAb and TmAb in GO and GD without ophthalmopathy(P〈0.01).Conclusion:TNF-alpha and IL-6 are involved in the pathogenesis of GD.The manifestation of IL-6 overexpression indicated a predominant Th2 mediated humoral immunity in GD without ophthalmopathy.In GO patients,Both Th1-derived TNF-alpha and Th2-derived IL-6 are upregulated relatively,TNF-alpha/IL-6 ratio show no significant difference between in GO group and control group(P〉0.05),suggesting Th0-Type immune responses may play a role in the development of GO.
