摘要
目的分析三种方法固定SchatzkerⅥ型胫骨平台骨折的生物力学效果,以指导临床应用。方法将24具新鲜成人尸体膝关节制成SchatzkerⅥ型胫骨平台骨折模型标本。随机分为三组,每组8具标本,分别为A组外侧支撑钢板辅以后内侧防滑钢板固定,B组外侧支撑钢板辅以前内侧限制接触性动力加压钢板(LC-DCP)固定,C组外侧支撑钢板辅以内侧半针外固定架固定。对不同内固定组进行抗载荷、抗扭转和抗剪切性力学性能测试。结果B组抗载荷、抗扭转和抗剪切性力学性能优于A组,C组最差,统计显示A、B组之间差异无统计学意义,而B、C组之间差异有统计学意义。结论B组外侧支撑钢板辅以前内侧LC-DCP固定组生物力学性能好,对于固定SchatzkerⅥ型胫骨平台骨折有临床意义。
Objective To compare the biomeehanical effect of three fixation instruments for Schatzker Ⅵ tibial plateau fractures. Methods Twenty four fresh knee specimens were made into models of Schatzker Ⅵ tibial plateau fractures. The Specimens were divided randomly into three groups. Group A were fixed with a lateral periarticular plate and a posteromedial antiskid plate, and group B with a lateral periarticular plate and an anteromedial limited contact dynamic compression plate (LC-DCP), and group C with a lateral periarticular plate and a medical external fixator, Each model was tested for its biomechanical performance in resisting compression, bending and rotation. Results The performance of group B was higher than group A in anti-compression,anti-bending and anti-rotation, and group C was the poorest, there was no significant difference between group A and group B, and there was signifi- cant difference between group B and group C. Conclusion The biomeehanical performance of group B was good, The method might have clinical applications in the treatment of Schatzker Ⅵ tibial plateau fractures
