摘要
目的探讨HLA—A、HLA—B、HLA—DRB1位点基因与山西汉族激素抵抗肾病综合征(SRNS)的相关性。方法用聚合酶链反应序列特异性引物法,对30例SRNS患者(成人22例,儿童8例)和45例正常对照者进行了HLA—A、HLA—B、HLA—DRB1等位基因分型,并分析了A、B、DRB1基因在各组的分布。结果SRNS患者组HLA—B+15、B+44基因频率较正常对照组增高(P〈0.05);成人SRNS患者组HLA—DRB1*07、B*44基因频率较正常对照组增高(P〈0.05),成人SRNS患者组HLA—DRBl}15基因频率较正常对照组降低(P〈0.05);儿童SRNS患者组HLA—DRB1*10基因频率较正常对照组增高(P〈0.05)。结论SRNS发病可能与HLA—B*15、B+44基因有关,成人SRNS发病可能与HLA-DRB1*07、B*44基因有关,HLA—DRB1*15对成人SRNS发病可能有保护作用,儿童SRNS发病可能与HLA—DRB1*10基因有关;HLA与SRNS的相关性不仅与人种、国家和地区有关,还可能与发病年龄有关。
Objective To investigate the predisposing alleles of HLA-A, HLA-B and HLA-DRB1 genes in Han chinese with steroid-resistant nephrotic syndrome (SRNS). Methods Polymerase chain reaction-spe- cific sequence primers (PCR-SSP) method was used to type HLA-A, HLA-B and HLA-DRB1 alleles in 30 patients with SRNS, which includes (22 adults and 8 heathy children) , and 45 control of heathy Han chineses from Shanxi province. Results The allele frequencies of HLA-B * 15 and HLA-B * 44 were higher in SRNS patients than those in normal controls ( P 〈 0.05 ). The allele frequencies of HLA-DRB1 * 07 and HLA-B * 44 were higher in adults with SRNS than those in normal controls( P 〈 0.05 ) . The allele frequency of HLA-DRB1 * 15 was lower in adults with SRNS than those in normal controls( P 〈 0.05 ) and the allele frequency of HLA-DRB1 * 10 was higher in children with SRNS than those in normal controls(P 〈0.05 ). Conclusion The results suggest that there may be association between SRNS and HLA-B * 15, HLA-B * 44, between adults with SRNS and HLA-DRB1 * 07 and HLA-B * 44 and between children with SRNS and HLA-DRB1 * 10. Possibly HLA-DRB1 * 15 allele plays a protect role in adults with SRNS and HLA associations with SRNS is not only related with area and race but also with age of disease onset.
