摘要
目的探讨膝关节骨性关节炎行关节镜清理术结合透明质酸关节腔内注射的疗效。方法2000年1～2007年1月间为150例(168膝)膝关节骨性关节炎患者行关节镜下关节清理术及透明质酸关节腔内注射治疗,并采用Lysholm评分法对术前、术后及随诊期间关节功能情况进行评价。结果术后Lysholm评分(85.0±7.2)、(80.6±8.1)与术前(53.8±6.9)相比有统计学差异(p<0.05),其中128例(135膝)随访1年资料完整,随访率85.3%,术后1年Lysholm评分(80.6±8.1),与术前评分值差异有统计学意义。其中术后及随诊1年疼痛单项评分较之术前亦有统计学差异。结论关节镜下清理术结合透明质酸关节腔内注射对膝关节骨性关节炎近、远期效果均佳,可作为该类患者的较好选择。
Objective To investigate the treatment and outcome of osteoarthritis of knee by arthroscopic arthroscopic debridement combined with hyaluronate sodium intraarticular injection. Methods During the period between Jan. 2000 and Jan. 2007, 150 patients（ 168 knees）with osteoarthritis were treated by arthroscopic debridement combined with hyaluronate sodium intraarticular injection in our hospital. The patients were assessed by the Lysholm score during pre - and postoperative and 1 year following up. Results One hundred and forty -two patients were followed up for at least one year. Lysholm score of the 150 patients showed a significant improvement from preoperative （53.8 ± 6.9） to postoperative （ 85.0 ± 7.2） and （ 80.6± 8.1 ） during following - up（p 〈 0.05 ）. Furthermore, the difference was significant when comparing the pain score of postoperative and 1 - year postoperative to preoperative （p 〈0. 05）. Conclusion As the treatment of osteoarthritis of knee, arthroscopic surgery showed the superiority to maintain good function of knee and may be the best treatment before total knee replacement.
