摘要
目的:探讨孕妇血清和羊水白细胞介素-8(IL-8)、肿瘤坏死因子-!(TNF-α)水平与胎膜早破合并宫内感染的相关性和临床意义。方法:选取我院2005年1月至2006年12月住院胎膜早破孕妇52例(研究组),以及产科门诊定期产检的正常孕妇50例为对照组;采用酶联免疫法(ELISA)测定血清与羊水IL-8、TNF-α水平;同步取胎盘胎膜组织进行病理组织学检查。结果:研究组血清和羊水IL-8水平均高于对照组(P<0.05),而研究组TNF-α仅羊水水平高于对照组(P<0.05)。研究组绒毛膜羊膜炎的发生率高于对照组(37%vs4%,P<0.01)。羊水IL-8水平与血清IL-8水平呈较好的正性相关(r=0.677,P<0.01);羊水TNF-α与羊水IL-8水平间亦有较好的正性相关(r=0.788,P<0.01)。结论:血清和羊水IL-8水平、羊水TNF-α水平与胎膜早破合并宫内感染有明显的正相关性,其中羊水IL-8的相关性最好。孕妇羊水IL-8、TNF-α水平可作为早期诊断胎膜早破合并宫内感染的指标。
Objective To investigate the relationship between interleukin-8 (IL-8), tumor necrosis factor-or (TNF-α) levels and premature rupture of membrane (PROM) in pregnant women with intrauterine infection. Methods From Jan. 2005 to Dec.2006, 52 pregnant women being in hospital with PROM and 50 normal term pregnant women in obstetric clinic as controls were recruited for study from the gynaecology and obstetrics department of our hospital. Both IL-8 and TNF-α levels in maternal serum and amniotic fluid were measured by enzyme-linked immunosorbent assay (ELISA). Chorioamnionitis was diagnosed histopathologically after labor by detected placentas. Results Both maternal serum and amniotic fluid IL-8 levels in PROM group were higher than those in control group (P 〈 0.05), but only the amniotic fluid TNF-α level in PROM group was higher than that of control group (P 〈 0.05). The incidence rate of chorioamnionitis in PROM group was higher than that in control group (37% vs 4% ,P 〈 0.01 ). There was higher positive correlation between amniotic fluid IL-8 level and maternal serum IL-8 level (r=0.677, P 〈 0.01). Moreover, there was significant positive correlation between TNF-α level and IL-8 level in amniotic fluid measurements (r=0.788,P 〈 0.01). Conclusions There is significant positive correlation between serum and amniotic fluid IL-8 levels, amniotic fluid TNF-α levels and PROM with chorioamnionitis. Therefore anmiotic fluid IL-8 is the best biomarker among these measurements for early identification of PROM with chorioanmionitis.
