摘要
目的探讨超声引导套管针穿刺微量胸腔积液治疗技术及其临床应用价值。方法在超声引导下置入套管针软管并使软管前端位于胸腔积液低位,在实时超声监视下完成积液的抽吸、冲洗及药物注射治疗。结果35例微量胸腔积液患者共行超声引导套管针穿刺置管77次,均一次穿刺成功,抽出液体最少15 m l,最多267 m l,平均124 m l。无一例出现并发症。抽尽液体后超声显示胸腔无积液。结论超声引导套管针穿刺治疗微量胸腔积液,方法简便、安全、成功率高,且无放射损伤,与其他影像学方法介入治疗相比有明显优势,推广应用此项技术将为胸腔积液的早期诊断和早期治疗提供强有力的技术支持。
Objective To study the value of ultrasound - guided thoracocentesis with thoracic drainage trocar in patients with micro - pleural effusion. Methods 35 patients with micro - pleural effusion identified by ultrasonography were managed by using ultrasound - guided thoracocentesis. The key of this technique is to place the tip of the trocar into the pleural effusion as low as possible. The whole process of aspiration, douche and remedy injection was supervised under ultrasound. Results The 77 performances of thoracocentesis in the present study were all successful. The amount of aspirated effusion ranged from 15 ml to 267 ml ( mean 124 ml) ; ultrasonography showed the chest was free from effusion after aspiration. No complications were encountered. Conclusion Ultrasound - guided thoracocentesis using thoracic drainage trocar is a convenient and safe technique of high rate of success for patients with micro - pleural effusion. Its ap- plication benefits early etiological diagnosis and treatment of pleural effusion.
