摘要
目的研究小儿柯萨奇病毒感染发病状况及其临床特点,探讨干扰素对其治疗作用。方法对190例患儿采用酶联免疫法检测柯萨奇病毒混合型IgM。并将CV-IgM阳性上呼吸道感染(上感)、肺炎患儿均随机分为2组,干扰素组在对照组治疗的基础上加用干扰素注射液肌注。结果①上感组、下呼吸道感染组(肺炎组)、病毒性心肌炎组、病毒性脑炎脑膜炎组阳性率分别为25.35%,24.62%(23.81%),38.46%,20.00%。②CV-IgM阳性患儿6—8月最多,0~6岁最多,与其他季节及年龄相比均有显著性差异(P〈0.05)。③CV-IgM阳性上感患儿干扰素组热退时间低于对照组,有显著性差异(P〈0.05)。CV-IgM阳性肺炎患儿干扰素组热退时间与对照组相比,无显著性差异(P〉0.05)。干扰素组治愈时间短于对照组,有显著性差异(P〈0.05)。结论目前柯萨奇病毒已成为小儿呼吸道感染的常见病原,其引起的疾病以病毒性心肌炎、急性上呼吸道感染、肺炎、病毒性脑炎脑膜炎为主。柯萨奇病毒感染多见于6岁以下小儿,夏季多见,干扰素治疗效果较好。
Objective It is to explore the characteristic of coxsackie virus (CV) infection in children and to explore the treating function of interferon on it. Methods IgM of mix type of CV was determined with euzymelinked immunosorbent assay in 190 cases of child patients. The patients with upper respiratory tract infection or pneumonia whose CV - IgM were positive were randomly divided into 2 groups. Interferon group was given interferon injection by intramuscular injection on the basis of treatment of control group. Results The positive rate of CV - IgM in upper respiratory tract infection group, lower respiratory tract infection group (pneumonia group), viral myocarditis group, viral encephalitis group respectively were 25.35 %, 24.62 % (23.81% ), 38.46 %, 20.00 %. Positive CV - IgM most appeared in June - August and in patients from 0 to 6 years old. Compared with other season and age, the differences were significant ( P 〈 0.05). Fever duration of upper respiratory tract infection patients with positive CV- IgM in interferon group was shorter than that in control group. The difference was significant (P 〈 0.05). There was no significant difference in the duration between control group and pneu- monia patients with positive CV- IgM in interferon group (P 〉0.05). The curing time of interferon group was shorter than that in control group, the difference was significant ( P 〈 0.05 ). Conclusion CV is a frequent pathogen which induces respiratory tract infection including viral myocarditis, acute upper respiratory tract infection, pneumonia, viral encephalitis in children. CV infection mostly appears in summer and in children from 0 to 6 years old. Interferon has a satisfactory effect on it.
