摘要
目的观察严重虹膜和晶状体损伤后Ⅰ期和Ⅱ期植入带虹膜隔人工晶状体的临床疗效。方法10例严重虹膜和晶状体损伤(脱位、缺如等)的患眼,手术治疗创伤后,Ⅰ期(3例)和Ⅱ期(7例)睫状沟位植入或缝线悬吊固定植入带虹膜隔人工晶状体,术后1 d,7 d,1个月,3个月观察术眼视力、切口、前房、人工晶状体位置、眼底、眼压。结果除1例Ⅰ期植入的患眼因术后前房出血和高眼压,于术后10 d 原切口取出人工晶状体外,其余9例患眼视力明显提高,术后3个月最佳裸眼视力0.5,最佳矫正视力0.6,患眼无畏光、眩光等视物不适症状。I期植入的患眼术后葡萄膜炎反应较Ⅱ期植入的患眼严重,且消退较慢。结论Ⅰ期植入带虹膜隔人工晶状体术后反应较重,应严格掌握适应证;Ⅱ期植入带虹膜隔人工晶状体适用于严重虹膜和晶状体损伤患眼的手术治疗。(中国眼耳鼻喉科杂志,2007,7:228-229)
Purpose To observe the clinical effect of Ⅰ-phase and Ⅱ-phase black-diaphragm intraocular lens implantation for treatment of traumatic (partial) aniridia combined with aphakia. Methods Ten cases of traumatic ( partial) aniridia combined with aphakia eyes were included in this study. Black-diaphragm intraocular lens implantation was performed immediately after eye injury (3 eyes) or 3-6 months after eye injury (7 cases). Post-surgery visual acuity, healing of the incision, anterior chamber, position of the lens, retina and intraocular pressure were examined. Results The lens was withdrawn 10 days after surgery in one case because of anterior chamber hemorrhage and high intraocular pressure ; in the rest of nine cases, visual acuity improved significantly, the best natural visual acuity reached 0. 5 three months after surgery, the best corrected visual acuity reached 0. 6. There was no photophobia or dazzle after surgery. More severe post-surgery uveitis was observed among those who received black-diaphragm intraocular lens implantation immediately after eye injury than those receiving surgery 3-6 months after injury. Condusion The post-surgery reaction of black-diaphragm intraocular lens implantation immediately after eye injury is quite heavy and the indication for surgery should be selected very carefully. Black-diaphragm intraocular lens implantation 3-6 months after eye injury was clinically effective and feasible for treatment of traumatic (partial) aniridia combined with aphakia.
