摘要
目的探讨宫内发育迟缓(IUGR)儿出生后脐血胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白3(IGFBP3)、胰岛素(Ins)及瘦素(Lep)的变化。方法选择141例IUGR儿为IUGR组,以同期出生的足月、适于胎龄儿30例作为对照组,比较两组出生后脐血IGF-1、IGFBP3、Ins和Lep的浓度。结果IUGR组IGF-1、IGFBP3、Ins和Lep的浓度均明显低于对照组(P值分别<0.01、0.05)。IUGR组IGF-1与IGFBP3(r= 0.6434)和Ins(r=0.4367)、Ins与Lep(r=0.3461)均呈正相关(P值均<0.01),IGF-1与Lep、IGFBP3与Ins和Lep均不相关(P值均>0.05)。结论IGF-1、IGFBP3、Ins及Lep低下是导致IUGR的原因, Ins-IGF-1-IGFBP3及Ins-Lep轴各自调控胎儿的生长发育。
Objective To investigate the change of serum levels of insulin-like growth factor 1 (IGF-I), insulin-like growth factor-binding protein-3 (IGFBP3), leptin and insulin levels in intrauterine growth retardation (IUGR). Methods Levels of serum IGF-I, IGFBP3, Leptin and insulin in the cord blood were measured in 141 IUGR and 30 normal newborns. Results Levels of serum IGF-I, leptin of IUGR were significantly lower than those of normal newborns (P 〈 0.01) ; the levels of IGFBP3, insulin of IUGR were also significantly lower than those of the normal newborns (P 〈 0.05). Serum. IGF-I was significantly correlated with IGF-BP3 and insulin levels (P 〈 0.05), but not with [eptin (P 〉 0.05). IGF-BP3 level was not correlated with Leptin and insulin level (P 〉 0.05). Insulin had significant relevance with leptin (P 〈 0.05). Conclusion The low serum levels of insulin, IGF-I, IGFBP3 and leptin can result in development of IUGR. Insulin-IGF-I-IGFBP3 and insulin-leptin controls the growth and development of the fetus. (Shanghai Med J, 2007, 30:425-427)
