摘要
目的探讨经颅硬膜外入路视神经管减压治疗外伤性视神经损伤12例(15侧)的经验。方法额下入路8例,眶额入路3例,眶上锁孔入路1例。于额部硬膜外显露前颅底,全程减压视神经。结果9例(12侧)术后1个月视力得到改善,无术后并发症。术前4例无光感,术后1月2例可见眼前手动。结论经颅硬膜外入路视神经管减压术安全有效,药物治疗无效后应积极行视神经管减压术,该方法为外伤后失明患者的有效治疗方法。
Objective To report the results of optic canal decompression via the transcranial epidural approach for the treatment of traumatic optic nerve injury in 15 sides of 12 cases. Methods The operation was performed on 8 cases via the subfrontal approach, 3 cases via the orbital-fi-ontal approach and 1 case via the supraorbital keyhole approach. Optic canal decompression was performed through revealing the anterior skull base via the trans-frontal epidural approach. Results Postoperative observation found that visual acuity of 9 cases (12 sides) was improved, with no postoperative complications. Among the 4 cases without light perception before the operation, 2 cases could sense the motion of hands 1 month after the operation. Conclusion Optic canal decompression via the transcranial epidural approach is effective and safe. If medication is ineffective, optic canal decompression should be performed actively, as an effective therapy for patients with ablepsia after trauma.
