RT—PCR检测胃癌患者外周血中细胞角蛋白20mRNA及其意义

摘要 目的检测胃癌患者外周血中细胞角蛋白20(CK20)mRNA的表达情况,并探讨其存在的临床意义。方法应用逆转录-聚合酶链反应(RT—PCR)检测56例胃癌(胃癌组)、18例良性胃病(良性胃病组)、20例正常献血者(对照组)外周血中CK20 mRNA的表达情况。结果对照组及良性胃病组外周血中CK20 mRNA均为阴性表达,胃癌组外周血中阳性表达率为35.7%。胃癌组外周血中CK20 mRNA的阳性表达率在不同肿瘤组织学类型、TNM分期之间差异有统计学意义(P〈0.05),在不同肿瘤部位及患者性别之间差异无统计学意义(P均〉0.05)。结论胃癌患者外周血中CK20 mRNA的阳性表达可作为肿瘤细胞血行播散的标志,有助于胃癌预后的判断。 Objective To detect cytokeratin 20 (CK20) mRNA in peripheral blood of the patients with gastric cancer and study its clinical significance. Methods Reverse transcriptase - polymerase chain reaction (RT- PCR) technique was used to detect the CK20 mRNA in peripheral blood of 56 patients with gastric cancer(gastric carcinoma group), 18 patients with benign gastric disease( benign gastric disease group) and 20 cases of control group. Results The expression of CK20 mRNA in peripheral blood of control group and benign gastric disease group were all negative. The expression of CK20mRNA in peripheral blood of gastric carcinoma group was 35.7%. The positive detection of CK20 mRNA in peripheral blood was significantly correlated with tumor histological types and TNM staging, but had no coincidence with tumor position and sex. Conclusions The expression of CK20 mRNA is a helpful marker of circulating dissemination of gastric cancer cells. It also can be helpful to judge the prognosis of gastric cancer.
