大肠癌淋巴结中CEA、CK20和MMP-7mRNA表达诊断肿瘤微转移的意义 被引量:4

The expression of CEA, CK20 and MMP-7 mRNA in lymph nodes of colorectal cancer and its relation to cancer micrometastasis

摘要 目的探讨大肠癌淋巴结中癌胚抗原(CEA)、细胞角蛋白(CK20)和基质金属蛋白酶-7(MMP-7)mRNA的表达及其与肿瘤微转移的关系。方法分别用RT-PCR技术检测CEA、CK20、MMP-7基因的mRNA在106例大肠癌患者628个淋巴结及18例良性肠道疾病患者45个淋巴结中的表达情况,并与常规病理学检查进行比较。结果大肠癌患者淋巴结中CEA、CK20、MMP-7mRNA表达阳性率分别为41.6%(261/628)、42.7%(268/628)和56.1%(352/628),病理学检查转移阳性率为22.3%(140/628);CEA、CK20、MMP-7mRNA的阳性检出率均明显高于病理学阳性检出率(均P<0.01)。而18例良性肠道疾病患者45个淋巴结三者表达均阴性。结论RT-PCR技术检测淋巴结CEA、CK20、MMP-7mRNA的表达可用于诊断大肠癌的微转移,其结果较常规病理学检查敏感,对临床分期、预后评估具有重要意义。 Objective To investigate the expression of CEA, CK20 and MMP-7 mRNA in lymph nodes of colorectal cancer and its association with cancer micrometastases. Methods CEA, CK20 and MMP-7 mRNA was detected using nested reverse transcriptase polymerase chain reaction (RT-PCR) in 628 lymph nodes from 106 patients with colorectal cancer and 45 lymph nodes from 18 patients with benign intestine diseases, and the results were compared with that of routine pathological examination. Results The positive rate of CEA mRNA, CK20 mRNA, MMP-7 mRNA in 628 lymph nodes were 41.6% (261/628), 42.7%(268/628), and 56.1%(352/628) respectively, while positive rate of pathological sections was only 22.3% (140/628). There were no significant differences between the positive expression rates of CEA mRNA and CK20 mRNA (P〉0.05), however both were lower than that of MMP-7 mRNA (both P〈0.01). The expression of CEA mRNA, CK20 mRNA, MMP-7 mRNA in 45 lymph nodes from 18 cases of benign intestinal diseases were all negative. Conclusion The expression of CEA -mRNA, CK20 mRNA, MMP-7 mRNA in lymph nodes can be used for diagnosing the micrometastases of colorectal cancer, it is more sensitive than the routine pathology histology.
