阻塞性睡眠呼吸暂停低通气综合征患者红细胞天然免疫功能的变化

Changes of erythrocyte innate immune function in patients with obstructive sleep apnea hypopnea syndrome

摘要 目的 研究阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者红细胞天然免疫黏附活性(erythrocyte innate immune adhesion function,EIIAF)的变化及和红细胞补体Ⅰ型受体(erythrocyte complement receptor type 1,C3bR,CD35,ECR1)数量变化之间的关系,探讨引起红细胞天然免疫功能变化的可能原因。方法 采用郭峰法和流式细胞仪间接免疫荧光法测定33例OSAHS患者和30例正常对照者外周血红细胞CR1活性及其数量。结果 OSAHS患者红细胞C3b受体花环率(RBC-C3bRR)(9.6±2.1)%明显低于正常对照者(11.2±2.2)%(P值〈0.01),红细胞免疫复合物花环率(RBC-ICR)(13.9±4.2)%明显高于正常对照者(9.7±2.1)%(P值〈0.01),红细胞CD35数量(ECD35)(41.4±7.6)%低于正常对照者(47.7±8.1)%(P值〈0.01),且OSAHS患者RBC-ICR水平与平均血氧饱和度(MSaO2)、最低血氧饱和度(LSaO2)、血氧饱和度〈90%时间占总睡眠时间百分比(SIT90)均有显著相关性(r=-0.438、-0.485、0.494,P值〉〈0.05、0.01、0.01),ECD35水平与MSaO2、LSaO2、SIT90有显著相关性(r=0.383、0.542、-0.395,P值〈0.05、0.01、0.05)。结论 OSAHS患者红细胞天然免疫功能下降,原因可能与夜间反复缺氧有关。 Objective To investigate the erythrocyte innate immune function in the patients with obstructive sleep apnea-hypopnea syndrome(OSAHS). Methods Complement receptor type 1 on erythrocyte (ECR1) in the fresh peripheral blood was detected in 33 OSAHS patients and 30 normal controls by Guo-Feng methods and FCM technique respectively. The immune function of red blood cells was detected, including the rossett rate of red-cell C3b receptors(RBC-C3bRR), red-cell immune complex(RBC-ICR) and the expression level of erythrocyte CD35. Results Compared with those of control group,the levels of RBC- C3bRR and ECD35 in OSAHS patients were significantly lower,respectively (9.6±2.1)M vs (11.2±2.2)%( P 〈0.01), (41.4±7. 6) % vs (47. 7±8. 1) % ( P〈0.01) ,and the level of RBC-ICR was significantly higher ( P 〈 0.01). In the patients with OSAHS, RBC-ICR level correlated significantly with MSaO2, LSaO2, SIT90( r = -0.438, -0. 485,0. 494, P 〈0.05,0.01,0.01), ECD35 level correlated significantly with MSaO2, LSaO2, SIT90 ( r=0.383,0.542,-0.395, P 〈0.05,0.01,0.05). Conclusions OSAHS patients show impairment of the erythrocyte innate immune function,which may be caused by hypoxia.
