摘要
目的讨论注射法胶原酶化学溶解术和椎间盘镜髓核摘除两种微创技术治疗腰椎间盘突出症的有关问题,旨在为临床工作提供指导。方法随机抽取同期应用上述两种方法治疗病例各200例,对其临床疗效、住院费用、住院天数、适应证及禁忌证等问题进行比较分析。结果两种治疗方法总有效率分别为86.7%、94.3%,平均住院费用分别为3816元、9837元,住院天数分别为5.5天、6.5天。后者适应证较前者广泛,禁忌证较少。结论上述两方法治疗腰椎间盘突出症均具有创伤小、出血少,脊柱力学结构完整,住院时间短、恢复快、患者痛苦小及乐于接受等优点,微创技术是治疗腰椎间盘突出症的有效手段,临床上可根据条件选择使用。
Objective To discuss the problems of treating for lumbar disc herniation with collagenase chemonucleolysis and microendoscopic discectomy. And give the help for clinic. Methods Rondomly selected 400 patients who received either of the above two treatments at the comparatively similar time in our hospital,with 200 cases in each group. Compared two methods clinic theropeutic efficenly,costs,time of hospitalization,indications and contraindications. Results The rate of efficiency of above two ways was 86.7% and 94.3%,average cost was 3816 and 9837 yuan,average time of hospitalization were 5.5 days and 6.5 days respectively. As for the indication and contraindication,microendescopic discectomy is suitable for more patients than collagease chemonucleolysis and its contraindication is less. Conclusion Both of the two methods have the advantages of less trauma,less bleeding,good spine stability after operation,shorter hospitalization,fast recovery,less suffering and easier to be accepted by patients,as a modem microtranmatic treatment for lumbar disc herniation,we suggest it might be used according to patient＇s conditions.
