摘要
目的探讨原发性食管小细胞癌的临床特点、治疗及预后。方法回顾性分析34例食管小细胞癌患者临床资料,按治疗方法分为综合治疗组和局部治疗组,比较各组的生存率,同时将可能影响预后的因素进行单因素、多因素分析。结果全组中位生存时间11个月,1、2、3年生存率分别为41.0%、26.3%、17.8%。综合治疗组和局部治疗组患者3年生存率分别为43.5%,0。全组6例死于局部肿瘤复发,4例死于淋巴结转移,17例死于内脏转移。单因素分析表明肿瘤直径、肿瘤分期、治疗方法明显影响预后(P<0.05)。多因素分析证实只有肿瘤分期、治疗方法是重要的预后因素(P<0.01)。结论原发性食管小细胞癌预后差,肿瘤分期和治疗方法是食管小细胞癌的预后因素,尽管手术联合化疗是较有效的治疗方法,考虑失败原因多为远处转移,应该进一步提高化疗的有效性。
Objective To investigate the clinical characteristics,management and prognosis of primary esophageal small cell carcinoma （PESCC）.Methods Thirty-four patients with PESCC including twenty-six men and eight females were analyzed retrospectively. The patients were divided into groups based on the method of treatment. The therapeutic effect and survival rate were compared between groups, prognostic factors were analyzed by the method of Log-rank and Cox Regression .Results The median survival time of all the patients was 11 months. The 1- year, 2- and 3-year survival rates were 41.0 %, 26.3 96 and 17.8 %, respectively. The 2-years survival rates were 43.5 % for the group treated by combined modality, and the 2-years survival rates were 0 for the group with local treatment. Six patients died of recurrence and 4 patients died of lymph node metastasis, others 17 patients died of hematogenous metastasis. Univariate statistical analysis showed that the length of tumor, the stage and type of treatment showed significance prognostic factors. In multivariate analysis, stage and type of treatment were shown to be independent predictive （P 〈 0. 01） . Conclusion Both tumor stage and type of treatment are possible prognostic factors. Because the mode of failure was remote metastasis, though surgery combined with chemotherapy may improve the treatment results for PESCC, it was still necessary to continue research the effect of chemotherapy .
