The Influence of a Muscle Relaxant Bolus on Entropy Values During Propofol Induced Loss of Consciousness

摘要 目的探讨肌松药对麻醉诱导意识消失期间熵指数的影响。方法30例行腹腔镜胆囊切除术的患者,随机分为2组:试验组给予单次维库溴铵,而对照组给予生理盐水。全麻诱导:2组均接受了目标浓度的丙泊酚输注直到病人意识消失。意识消失后的2分钟,试验组给予单次维库溴铵0.1mg/kg,对照组给予生理盐水。记录脑电双频指数及状态熵和反应熵指数。结果在意识消失时的两组间结果相似。病人意识消失后对照组用药后较意识消失时的脑电双频指数(P<0.002),状态熵(P<0.05),反应熵(P<0.01)均下降。试验组在脑电双频指数(P<0.0001),状态熵(P<0.0001),反应熵(P<0.0001)下降。试验组在双频指数(P<0.05)和反应熵(P<0.01)方面的下降较对照组显著。结论浅麻醉病人给予肌松剂时双频指数和反应熵下降,而状态熵则变化不明显。 Objective To investigate the influence of muscle relaxants on entropy values during propofol induced loss of consciousness. Methods Thirty patients ASAⅠ-Ⅱ ,scheduled for elective laparoscopic surgery, were randomly allocated to received a target-controlled infusion of propofol, titrated until loss of consciousness. Two minutes after loss of consciousness, the study group received a bolus injection of vecuronium, whereas the control group received a placebo(Saline). The following variables were recorded: bispectral index, state entropy, and response entropy. Results Similar values were obtained in both groups at loss of consciousness. Saline administration induced a decrease in bispectral index (P〈0. 002), state entropy (P〈 O. 05), and response entropy (P〈0. 01) compared with the values measured at loss of consciousness. Vecuronium administration induced a decrease in bispectral index (P〈 0.0001), state entropy (P〈 0.0001), and response entropy (P〈 0.0001) values. Decreases in bispectral index (P〈 0. 05) and response entropy (P〈 0.01) were larger after vecuronium than placebo injection. Conclusion In lightly anesthetized patients, myorelaxant administration decreases bispectral index and response entropy, but not state entropy values.
