摘要
目的观察皮肌炎合并间质性肺损害的临床特点,提高临床医生对皮肌炎肺损害的认识。方法回顾性分析1993年1月~2004年12月本科皮肌炎合并肺损害患者的一般资料、诊断、治疗及转归情况。结果患者的临床表现以发热、咳嗽、咳痰、呼吸困难、胸痛为主要症状,皮肌炎肺损害症状不典型,本组26例中误诊8例,误诊率31%,死亡8例,病死率31%。结论皮肌炎肺损害症状不典型,体征不明显,易误诊、漏诊,如不及时诊治,常会导致严重后果。
Objective To present a summing-up material of dermatomyositis combined with interstitial pneumonia; to inprove the clinicians understand of lung impairment caused by dermatomyositis. Methods To make a summary by reviewing the average materials and cases of diagnosis, treatment and recovery of lung impairment of dermatomyositis patients in this department from Jan, 1993- Dec, 2004. Results The patients main symptoms are as follows. The patients often have a fever, have a cough sometimes with phlegm, feel difficult to breath and have pain in the chest. However, in most cases, the symptoms of the impairment of dermatomysitis（DM） are not easy for the doctors to identify. In my group, Of 26 cases, there are 8 cases misdiagnosed and 8 deaths. The rates of the results are the same,that is 31%. Conclusion The physical symptom of lung impairment of dermatomyositis was neither typical n^r apparent. It is very easy for the clinicians to mlsdiagnose this kind of disease or to neglect it. If this kind of disease is not diagnosed and be treated on time,it will bring about serous consequences .
