摘要
目的了解成人皮肌炎(adult dermatomyositis,ADM)患者的生活质量(quality of life,QOL)并分析其影响因素。方法应用皮肤病专用生活质量量表Skindex-29和健康状况调查问卷简表-36(SF-36)分别对42例ADM患者、40例健康者对照进行问卷调查,用两独立样本t检验比较两组QOL评分,以多元线性回归相关分析其影响因素。结果 ADM病例组Skindex-29评分在症状、功能、情感方面均高于健康对照组(P<0.01);ADM病例组在SF-36的所有8个维度的评分上均显著低于健康对照组(P<0.01),而合并内脏损害的ADM患者SF-36个维度评分低于无内脏损害未并发肿瘤的ADM患者(P<0.05);影响ADM患者QOL的主要因素为肌力、疼痛程度、皮损情况、瘙痒程度、年龄等因素的影响。结论 ADM患者QOL偏低,累及内脏合并肿瘤者QOL更差,通过提高患者肌力、缓解疼痛及皮肤症状可能有助于改善患者QOL,提示临床治疗中应关注患者心理变化,及时有效的心理疏导对提高患者QOL有重要帮助。
Objective To investigate the quality of life of Adult Dermatomyositis patients and analyze its influencing factors. Methods Shindex-29 （ dermatosis specific quality of life scale） and SF-36 （ medical outcomes study short form-36, SF-36） were used to carry out the survey among 42 ADM patients and 40 healthy controls. Two-sample t test was used to compare the scores in the quality of life between two groups and multiple linear regression analysis was used to screen influencing factors in the quality of life in ADM group. Results ADM group scored higher than healthy controls did in emotional dimension, symptoms dimension and function dimension of Skindex-29 （P 〈 0. 01 ） and lower than healthy controls did in all the 8 domains of SF-36 （ P 〈 0. 01 ）. The ADM patients who suffered from organ damage and concurrent tumors scored lower than simple ADM patients did in several domains of SF-36 （ P 〈 0. 05 ）. The main factors that affected the quality of life of AMD patients were myodynamia, pain degree, skin lesions, pru- ritus, age and so on. Conclusion The quality of life of ADM patients was poor and much worse in pa- tients who suffered from organ damage and concurrent tumors. The quality of life of ADM suffers can be improved by enhancing muscle strength, allaying myodynia and alleviating skin lesions and pruritus, etc. Doctors should pay attention to the psychological changes of patients during the treatment and some timely and effective psychological consultation can improve the quality of life of patients as well.
