摘要
目的探讨高频彩超检查乳腺良恶性肿块的声像图特征及诊断价值。方法对经病理证实的乳腺良性肿块82例和恶性肿块34例的高频彩超声像图进行回顾性分析。结果乳腺良性肿块大多表现为:形态规则,边缘光整,内部回声均匀,后方无衰减,纵横径比〈1;彩色多普勒血流分级大多数为0~Ⅰ级;PI<1.3;RI<0.70。乳腺癌的肿块多数表现为:形态不规则或欠规则,边缘不整,内部以低回声为主,部分后方有衰减,肿块纵横径比〉1;彩色多普勒血流分级大多数为Ⅱ~Ⅲ级;PI〉1.3;RI〉0.70;同侧腋窝淋巴结肿大,血流较丰富。结论高频彩超对乳腺肿块的检出率高,应用二维声像图特点与CDFI、PW定量及半定量分析,综合判断,可以明显提高对乳腺良恶性肿块的诊断和鉴别诊断水平。
Objective To explore the ultrasonic features and diagnostic value of breast tumors. Methods The features of HFUS were retrospectively analyzed in 82 cases with breast benigh tumors and 34 cases with malignant tumors identified with pathology. Results Most of the benigh tumors appeared as regular shape,smooth border, homogeneous internal echoes, no rear attenuation and length/width〈1. Doppler flow imaging grades were 0- Ⅰ , the pulsatility index (PI) 〈1.3 and the resistance index (RI) 〈0.70. The majority malignant tumors appeared as irregaler shape and border low echoic, no attenuation and length/width〉1. Doppler flow imaging grades were Ⅱ - Ⅲ with abandant blood flow, PI〈1. 3 and RI〉0.70. The ipsolateral axillary lymph nodes were enlarged. Conclusions The detection rate of HFUS was very high. Using 2D, CDFI and PW quantitative analysis, comprehensive judgement may improve the diagnosis and differential diagnosis of breast tumors.
