摘要
目的观察亚甲蓝甘油合剂封闭治疗三叉神经痛的临床疗效并探讨其治疗机制。方法按国际头面痛学会分类委员会确定的诊断标准,108例原发性三叉神经患者随机分为两组:A组为亚甲蓝甘油合剂组(n=54),B组为单纯甘油合剂组(n=54);采用徐贵印推荐的三维测角穿刺半月神经节进行封闭治疗,对治疗后1周、3个月和2年分别进行疗效评判和随访观察。结果两组治疗后1周、3个月优良率比较差异无统计学意义(P>0.05),两年后优良率比较差异有统计学意义(P<0.05);两组并发症没有明显差异,2例出现视力下降,3例因解剖变异未能穿刺成功,仅在卵圆孔周围进行了注射,外加眶下孔以及骸孔封闭。结论亚甲蓝甘油合剂用于治疗三叉神经痛远期疗效优于单纯甘油合剂,且亚甲蓝对神经未见明显损害(经2年随访)。
Objective To observe the clinical efficacy of Methylene blue - Glycerin mixture injection in treatment trigeminal neuralgia and evaluate the mechanism of the therapeutic effect. Methods According the diagnostic criteria of the headache classification committee of international headache society, 108 patients with trigeminal neuralgia were randomly divided into two groups. Group A： treated by Methylene blue - Glycerin mixture （ n =54 cases）,group B：treated by Glycerin mixture（ n =54 cases）. The way of directly puncturing forament ovale localized three -dimensionally on the face was used for injection. 7 days,3 months and 2 years respectively postoperatively the clinical efficacy was observed ,assessed ,compared and recorded on based of the effective criteria. Results The marked effective rate of group A（86. 55% ） is superior to group B（66. 7% ）2 years after treatment, the comparison group A and group B, paired X^2 test, P 〈 0. 05, there were significant differences in the marked effectiverate. Though the marked efffective rate of group A is a bit higher than group B 7 days and 3 months after treatment, there is not statistics meanings. The comparison （ group A and group B） of incidence of post postoperative complication and dys -response has no show the signifcant difference. No case died in group A and group B,the most serious complication is diminution of vision（2 cases）. All the patients finished the puncture and treatment successfully except 3 failed in dissection due to dissociation. Conclusion The injection of glycerin mixture with methylene -blue could raise the long -term effect and lower the recurrence rate compared with simple glycerin mixture injection,the evident never lesion was not observed during the follow -up 2 years.
