舌鳞状细胞癌手术安全切缘的分子生物学定量研究

摘要 目的 通过分子生物学检测,探讨舌鳞状细胞癌(简称鳞癌)黏膜安全的切除缘宽度。方法 通过翻译启动因子(eIF4E)对舌鳞癌及其周边的黏膜组织的检测,分析其距肉眼肿瘤边界不同距离黏膜的阳性表达率之间的关系,进行统计学检验。结果 舌鳞癌内部及距肉眼肿瘤边界0~5mm、5~10mm、10~15mm、15~20mm各组黏膜的eIF4E染色阳性率分别为100.00%、80.00%、63.33%、50%、13.33%。其中0~5mm、5~10mm、10~15mm 3组之间差异无显著性(P〉0.05)。而10~15mm、15~20mm 2组之间差异存在显著性(P〈0.05)。结论 舌鳞癌的黏膜切缘宽度应〉15mm。 Objective To explore the width of safe surgical margin of TSCC by molecular biological testing. Methods We analyzed the relations between positive expressions of mucosa at different distances from gross tumor edge by testing eIF4E on TSCC and its neighboring mucosa. All data were statistically tested with SAS6.12 software package.Results Positive rates of eIF4E staining inside TSCC and 0-5 mm, 5- 10 mm, 10-15 mm, and 15-20 mm of gross tumor border were 100%, 80%, 63.33%, 50% and 13.33%, respectively. There was no significant difference in groups of 0-5 mm, 5-10 mm and 10-15 mm( P 〉 0.05), but there was significant difference between groups of 10-15 mm and 15-20 mm (P 〈 0.05). Conclusion The width of surgical margin of TSCC mucosa should be at least 15 mm.
