摘要
目的探讨VEGF和nm23蛋白在膀胱移行细胞癌中的表达及其与肿瘤微血管密度(MVD)和侵袭转移的关系。方法应用免疫组织化学技术检测55例膀胱移行细胞癌VEGF、nm23和CD34蛋白的表达,并根据CD34染色结果计算肿瘤的微血管密度。结果55例膀胱移行细胞癌中,VEGF蛋白表达阳性率47.3%,其表达与脉管侵袭及其预后有显著性差异(P<0.05),VEGF阳性组MVD显著高于阴性组(P<0.01);nm23蛋白表达阳性率63.3%,其表达与脉管侵袭及其预后有显著性差异(P<0.05)。nm23阳性组MVD低于阴性组(P<0.05);VEGF与nm23的表达强度呈负相关(r=-0·647,P<0.01),VEGF阳性表达伴nm23阴性表达者易发生转移。结论VEGF可能是膀胱癌的主要促血管生成因子,和肿瘤的生成、发展及预后相关。联合检测VEGF和nm23蛋白可以作为预测膀胱癌转移及其预后的参考指标。
Objective To investigate the expressions of vascular endothelial growth factor ( VEGF), nm23 protein and intratumoral microvascular density(MVD) in transitional cell carcinoma (TCC) of the bladder and their correlation with metastasis and invasion. Methods The clinical data of 55 cases of TCC of the bladder from 1978 to 1992 were collected. Six specimens of normal bladder mucosa served as controls. The expressions of VEGF, nm23 and CD34 protein in TCC tissues and normal bladder mucosa were detected by immunohistochemistry. MVD was determined according to the result of CD34 protein staining. Results The positive rate of VEGF protein expression in TCC was 47.3%. VEGF expression was correlated to invasion, metastasis and prognosis of TCC. MVD in VEGF-positive cases was significantly higher than that of VEGF-negative cases (P 〈 0.01 ). The positive rate of nm23 protein expression in TCC was 63.6%. The positive rate of nm23 in TCC was of significant difference between the cases with vessel invasion and without. MVD of nm23-positive cases was significantly lower than that of nm23-negative cases (P 〈 0.05). The correlation analysis showed that VEGF expression was negatively related to nm23 expression in TCC(r =0. 647, P 〈0. 01 ). The cases with positive VEGF expression and negative nm23 expression were easy to develop metastasis. Conclusion VEGF may be one of main angiogenesis factors and was correlated with development, metastasis and prognosis of TCC. The combined detection of VEGF and nm23 may be a predictor for metastasis and prognosis of TCC.
