摘要
目的:观察加减陷胸桃承汤合参麦注射液对盲肠结扎穿孔术(CLP)后急性呼吸窘迫综合征(ARDS)模型大鼠的治疗作用,并探讨其机制。方法:采取CLP造成大鼠ARDS模型,分别以加减陷胸桃承汤(桃承组)、参麦注射液(参麦组)、加减陷胸桃承汤合参麦注射液(合方组)预处理动物,进行对照观察。于3、6、12和24 h采血进行血气分析,于3 h和12 h取肺组织检测细胞凋亡情况。结果:模型组随时间延长,动脉血氧分压(PaO2)逐渐下降,动脉血二氧化碳分压(PaCO2)逐渐上升,与治疗组及假手术组比较差异均有显著性(P<0.05或P<0.01)。用药各组PaO2均较模型组升高,但仍低于假手术组(P<0.05或P<0.01),其中参麦组与合方组回升幅度较桃承组大,差异均有显著性(P均<0.05);随着时间的延长,桃承组、参麦组和合方组之间PaCO2水平变化不明显,但均较模型组明显下降(P<0.05或P<0.01),与假手术组比较差异均无显著性(P均>0.05)。模型组细胞凋亡明显增加,与假手术组比较差异有极显著性(P均<0.01);用药各组细胞凋亡表达较模型组明显下降(P<0.05或P<0.01),与假手术组比较差异均无显著性(P均>0.05)。结论:加减陷胸桃承汤与参麦注射液均有升高CLP后ARDS模型大鼠PaO2、降低PaCO2的作用,且有一定的协同作用;其机制可能与其减轻肺组织细胞凋亡有关。
Objective: To explore the therapeutic effect of modified Xianxiong Taocheng decoction ( MXXTCD, 加减陷胸桃承汤) and Shenmai injection (参麦注射液) in acute respiratory distress syndrome (ARDS) rats induced by cecal ligation and performation, and to approach the pathogenic mechanism. Methods: ARDS model was induced by cecal ligation and performation. Rats were randomly divided into five groups: sham-operation group, model group, pretreatment groups which were pretreated with combined MXXTCD and Shenmai injection, MXXTCD only, and Shenmai injection only, respectively. At 3, 6, 12 and 24 hours after model was established, the arterial blood gas analysis of rats was performed, and at 3 and 12 hours, the apoptosis of lung tissue expression was measured respectively. Results: The level of arterial partial pressure of oxygen (PaOz) in model group decreased and arterial partial pressure of carbon dioxide (PaCO2) increased gradually followed the time, and the differences were significant compared to those of the other groups (P〈0. 05 or P〈0.01). PaO2 in pretreatment groups was higher than that of the model group but lower than that of the sham-operated group (P〈0. 05 or P〈0. 01), and the reversed extend of MXXTCD group combined Shenmai injection as well as Shenmai injection group was higher than that of MXXTCD group (both P〈0. 05). The changes of PaCO2in the pretreatment groups were not significant, and the difference was not obvious compared with the sham-operated group, but it was lower compared with the model group (all P〈0. 01). Apoptosis expression in the lung tissue was increase in model group (both P〈0. 01). The expression was little in sham-operation group. Levels of the other groups were notably different from model group, but had no significant difference compared with sham-operated group. Conclusion: MXXTCD and Shenmai injection can highten the level of PaO2 ,decline the level of PaCO2 and the rate of apoptosis ,abate lung injury.
