摘要
目的:观察α-氰基丙烯酸正丁酯(简称医用胶)作为充填物能否阻止手术后的骨桥复发,并初步探讨充填物在骺板部分早闭治疗中的作用。方法:实验于2004-05/2005-04在解放军总医院实验动物中心完成。通过切除兔股骨远端外髁处部分骺板,制成周围性骺板部分早闭的动物模型。在此基础上,采用28只4~6周的新西兰幼兔,随机分为4组,每组12只。对照组仅行关节囊切开,不切除骺板;模型组切除股骨远端外髁处部分骺板,但不充填任何物质;脂肪组充填自体脂肪;医用胶组充填医用胶。术后通过双下肢前后位X射线片、大体标本、组织学、组织化学等方法来观察和探讨医用胶治疗骺板部分早闭的效果。结果:实验兔28只均进入结果分析。①使用脂肪和医用胶作为切除部分骺板所遗留骨缺损腔的充填物,术后16周股骨的长度较模型组明显延长[(8.95±0.16),(9.12±0.25),(7.88±0.46)cm]。②术后16周股骨远端的外翻角度脂肪组和医用胶组及模型组分别为(19.6±4.86)°,(15.4±2.13)°,(46.2±3.42)°。③股骨远端生长长度对照组最长,约占整个股骨生长的57.6%,模型组最短,只相当于整个股骨生长的21.4%,脂肪组及医用胶组生长处于对照组与模型组之间,分别占整个股骨生长的46%和52%。结论:充填物的充填并不能阻止因切除部分骺板所激活的成骨过程,而只是作为一种空间占据物,占据了形成骨桥所需的空间。医用胶是作为充填物的很好的选择。
AIM: To observe whether N-Butyl cyancacrylate (medical glue for short) as filler is able to prevent the recurrence of physeal bridge after operation, and preliminarily investigate the effect of filler in the partial early-closure treatment on the epiphyseal plate.
METHODS: This experiment was carded out at Experimental Animals Center of General Hospital of Chinese PLA from May 2004 to April 2005. Based on a rabbit model of peripheral physeal arrest that was emulated by removing a plug of bone at the level of the epiphyseal plate on the lateral condyle on the distal femur, 28 white New-Zealand rabbits ,aged 4 to 6 weeks, were used and randondy divided into four groups, with i2 rabbits in each one. In control group, only articular capsule was exposed, and epiphyseal plate was not cut; In model group, partial epiphyseal plate at external condyle of distal femur was removed, . but no idler was given: autologous fat and N-Butyl cyanoacrylate were placed on the defects in fat group and medical glue group, respectively. The effect of medical glue in the epiphyseal plate partial early-closure .treatment was observed and investigated with X-ray of anteroposterior position of two inferior extremities, gross specimen, histological and histochemical methods. RESULTS: Totally 28 experimental rabbits entered the stage of result analysis. ① Using fat and medical glue as the filler of bone defect cavity left by cutting partial epiphyseal plate, the length of bone at postoperative 16 weeks was longer in fat group and medical group than in model group [(8.95±0.16), (9.12±0.25), (7.88±0.46) cm]. ②Ecstrophy angel of distal femur at postoperative 16 weeks was (19.6±4.86)°, ( 15.4±2.13 )°, (46.2 ±3.42)° in fat group, medical glue group and model group, respectively. ③The growth length of distal femur was the longest in control group, about 57.6% of the whole femoral length, and shortest in model group, only 21.4% of the whole femoral length.
