摘要
目的:研究医用胶作为充填物能否阻止手术后的骨桥复发.方法:采用28只4~6周的新西兰幼兔,随机分为四组,A组为对照组,仅行关节囊切开,不切除骺板;B组为模型组,切除股骨远端外髁处部分骺板,但不充填任何物质;C组充填自体脂肪,D组充填医用胶.术后通过双下肢前后位X线片、大体标本、组织学、组织化学等方法来观察和探讨医用胶治疗骺板部分早闭的效果.结果:脂肪组和医用胶组与模型组相比均能减轻股骨的纵向生长障碍及膝关节的成角畸形.医用胶对于减轻股骨远端的成角畸形更加有效.但自体脂肪和医用胶都不能使股骨的生长达到完全正常.结论:充填物的充填并不能阻止因切除部分骺板所激活的成骨过程,而只是作为一种空间占据物,占据了形成骨桥所需的空间.医用胶是作为充填物的很好的选择.
Objective: To examine whether medical clue was able to prevent the recurrence of physeal bridge after operation. Methods:Based on a rabbit model, 28 white New-Zealand rabbits, 56 legs,were used and divided into four groups randomly. In Group A, the femoral condyle was exposed, but no defect was created; in Group B, a defect was created but left unfilled; autologous fat and N-butyl cyanoacrylate were placed in the defect in Group C and D. 20 rabbits were radiographed weekly, and sacrificed 16 weeks after the operation,femora were removed, examined grossly and radiographed. Another 8 rabbits were killed at 5 weeks and 10 weeks, and histological, histochemical examinations were performed. Results:Compared with Group B, both antologous fat and medical glue can be used as interposition materials to make the femur grow longer and reduce the valgus deformity of knee joint, but N-butyl cyanoacrylate is more effective. Conclusion:None of the interposition materials, however, allow for completely normal growth, but N-butyl cyanoacrylate is a good choice to be used as the interposition material.
