摘要
目的研究胸腔积液端粒酶活性及CYFRA21-1在良、恶性胸腔积液中的诊断价值。方法用聚合酶链反应—酶联免疫吸附分析法(PCR-ELISA)检测胸腔积液端粒酶活性,用免疫放射分析法(IRA)测定胸液CYFRA21-1水平,并与细胞学诊断结果进行比较,根据最终的诊断结果,67例患者分成2组:(1)非恶性胸腔积液35例;(2)恶性胸腔积液32例。结果非恶性胸腔积液中端粒酶阳性2例(5.7%),恶性胸腔积液阳性26例(81.3%),端粒酶活性测定诊断恶性胸腔积液的灵敏度为81.3%,特异度94%,正确率93%。CYFRA21-1诊断灵敏度68.8%,特异度87%,正确率73%。端粒酶和CYFRA21-1均阳性者与细胞学诊断符合率57.0%。结论胸腔积液端粒酶活性和CYFRA21-1对鉴别良恶性胸腔积液均有一定的价值,联合检测能提高诊断的准确率。
Objective To study the diagnostic value of telomerase activity in differentiating malignant from nonmalignant pleural effusions, and CYFRA21-1. Methods Sixty-seven specimens of pleural effusion were examined for telornerese activity and CYFRA21- 1 by PCH-ELISA and IRA,respectively,According to the final diagnosis achleved, the pleural were classified into two groups;group Ⅰ ,35 nonmalignant pleural effusions,proup Ⅱ ,32 malignant plural effusions.Results Two of thirty-five（5.7% ）samples of plural effusions in group were found to possess weak telomerase activity and 26 of 32（81.3% ） samples in groups showed positive enzymatic activity. The diagnostic sensitivity is 81.3%, specificity is 94%, the overall accuracy is 92.3%. The diagnostic efficiency of CYFRA21-1 as following：sensltivity 68%, specificity is 87.9%, accuracy 73%. The eonsistant rate of telomerase and CYFRA21-1 with cytology was 57.02 %. Conclusion The assay of telomerase and CYFRA21 - 1 is valuable in differentiating benign from malignant effusions. The eombination of the two assays can increase the accuracy of the diagnosis.
