摘要
目的研究结核痛控制项目中的资金投入与产生的社会、经济效益的关系，为财政、卫生部门的卫生资源投入与使用提供科学依据。方法收集1993-2004年深圳市结核病控制项目资金的投入与结核病人诊断、治疗、管理等有关资料，采用成本-效果、成本-效用、成本-效益分析方法进行结核病控制项目卫生经济学评价。结果1993-2004年总治愈率达95．1％．每治愈1例活动性肺结核病人投入的费用为2857元；直接挽回DALY206 912年，间接挽回DALY89 982年；DOTS方案与传统方法比较治疗潦阳病人节省费用5029．6万元，控制传染源减少传染而节约费用5464．3万元。挽回GDP损失769712．6万元，避免因病造成的个人损失累计447757．6万元；户籍、暂住涂阳患病率年均递降分别为8．7％、5．3％；原发性耐药率由1999年35．6％下降至2004年的16．6％；获得性耐药率由1999年32．7％下降至2004年的29．5％。结论深圳市结核痛控制项目减轻肺结核患者的经济负担，避免大量的经济损失。取得了显著的社会效益和经济效益。
Objective To analyze the association of finance devotion with social and economic benefits of TB Control Project in Shenzhen and provide scientific reference for both finance and health departments to properly invest and use health resource. Methods Data concerning investment in TB Control Project in Shenzhen,diagnosis, treatment and management of pulmonary tuberculosis during the years of from1993 to 2004, were collected and the cost - effectiveness and cost - benefit were economically analyzed. Results From 1993 to 2004, the total cure rate was 95.1%. Cure expenses for one active pulmonary tuberculosis ease was RMB 2857 Yuan, DALY of 206 912 year was directly saved and DALY of 89 982 year was indirectly saved. When compared with traditional methods, DOTS saved RMB 4.8397 million Yuan for smear positive cases, and RMB 54.643 million Yuan was saved in reducing infection by controlling. In addition, the loss of GDP RMB 769.7126 million Yuan was reduced and a total individual loss from pulmonary tuberculosis of RMB 447.7576 million Yuan. The annual reduction rates of pulmonary tuberculosis in permanent and temporally residents8.7% and 5.3%. The primary resistant rate to antibiotics was reduced from 35.6% in 1999 to 16.6% in 2004. The acquired resistance rate was reduced from 32.7% in 1999 to 29.5% in 2004. Conclusion TB Control Project implemented in Shenzhen has reduced the economic burden, and economic loss and significant social and economic benefits have been achieved.
