摘要
目的:观察中药益肾通痹方对实验性兔膝关节骨性关节炎模型动物血清和关节腔液中白细胞介素1β、肿瘤坏死因子α、一氧化氮含量,以及膝关节软骨病理形态的影响,分析益肾通痹方防治膝关节骨性关节炎的免疫学机制。方法:实验于2004-08/12在新疆医科大学动物实验中心完成。①选用成年雄性新西兰兔24只。分别于实验第1,4,7天于兔右膝关节腔内注射40g/L木瓜蛋白酶0.3mL,建立膝骨关节炎模型。②将造模成功的24只兔按随机抽签法分为3组:模型组、益肾通痹方组、布洛芬组,每组8只。从造模后第15天开始,益肾通痹方组:灌胃益肾通痹方含生药7.2g/(kg·d)(主要成分狗脊、骨碎补、牛膝、鸡血藤、川芎等,自行配制)20mL;布洛芬组:灌胃布洛芬混悬液(中美史克制药有限公司生产,生产批号:04080081,300mg/片)28.44mg/(kg·d);模型组:灌胃生理盐水;1次/d,连续15d。③于实验第31天,采用光学显微镜(苏木精-伊红染色×100)观察软骨病理形态学变化。采用放射免疫分析法,根据由解放军总医院放免研究所提供的试剂盒检测血清及关节腔液中白细胞介素1β和肿瘤坏死因子α含量。按南京生物建成公司生产的一氧化氮试剂盒说明,测定血清及关节腔液中一氧化氮含量。④计量资料差异比较采用方差分析。结果:新西兰兔24只均进入结果分析。①病理形态观察结果:益肾通痹方组软骨及滑膜病变程度较其他2组有不同程度的减轻。②血清细胞因子水平:益肾通痹方组血清白细胞介素1β水平明显低于模型组(F=2.778,P<0.05),其余各组间血清各细胞因子水平差异不明显(P>0.05)。③关节腔液中细胞因子含量:益肾通痹方组关节腔液中白细胞介素1β、肿瘤坏死因子α、一氧化氮含量明显低于模型组(F=3.225~5.038,P<0.05~0.01)。布洛芬组与模型组,益肾通痹方组与布洛芬组关节腔液中细胞因子含量比较,差异不
AIM; To observe the effects of Yishen Tongbi prescription on contents of interleukiu-1β(IL-1β), tumor necrosis factor-α(TNF-α), nitric oxide (NO) in serum and joint cavity fluid of the rabbit experimental knee osteoarthritis, as well as pathologic picture of cartilage of knee joint, and to analyze immunologic mechanism of Yishen Tongbi in prevention and treatment of knee osteoarthritis.
METHODS: The experiment was performed at the animal experiment center of Xinjiang Medical University fi'om August to December 2004. ① Totally 24 New Zealand adult male rabbits were selected. 40 g/L caroid 0.3 mL was injected into joint cavity of right knee of rabbits at the 1, 4, 7 days, respectively to establish models with knee osteoarthritis. ②The 24 rabbit models that had been established successfully were randomly assigned into 3 groups: model group, Yishen Tongbi group, brufen group with 8 ones in each group. From the 15^th day after establishing, the rabbits in the Yishen Tongbi group were treated with gastric perfusion with Yishen Tongbi prescription that contained 20 mL 7.2 g/kg crude drug daily (mainly composed of cibot rhizome, rhizoma drynariae, achyranthis, caulis spatholobi, rhizoma and so on, made by oneself). Those in the brufen group were treated with gastric perfusion receiving 28.44 mg/kg ibuprofen suspension daily (produced by Zhongmei Shike Zhiyao Youxian Gongsi, number of production: 04080081, 300 mg per piece). Those in the model group were treated with' saline by gastric perfusion, once a day for 15 days. ③At the 31^th day light microscope (hematoxylin-eosin staining xl00) was applied to observe pathological change of cartilage. Using radioimmunoassay, the contents of IL-1β and TNF-α in serum and joint cavity fluid were determined on the basis of kit provided by Institute of Radio Immunity, General Hospital of Chinese PLA. Content of NO in serum and joint cavity fluid was detected according to the NO kit instruction designed by Nanjing jiancheng Bioengineering
