摘要 目的探讨血栓前体蛋白(TpP)、D-二聚体(D-D)测定在急性脑梗死早期诊断及治疗中的意义。方法对不同发病时间的急性脑梗死患者共55例分别于入院后立即以及常规治疗后测定血浆TpP、D-D含量,并与正常组比较。结果急性脑梗死患者血浆TpP〔(21.75±11.69)mg/ml〕、D-D〔(0.98±0.47)mg/ml〕水平显著高于正常对照组〔分别是(2.17±1.25)mg/ml,(0.25±0.17)mg/ml,P<0.01〕,D-D在发病后12h有显著升高,并随着发病时间的延长,浓度有升高的趋势,且较长时间维持高浓度。TpP水平在发病后6h内即可出现显著升高,有效治疗后TpP迅速下降。结论TpP、D-D不仅是急性脑梗死早期诊断的可靠指标,它还可用于监测疾病的发展及治疗效果的评价。 Objective To observe the relation between the changes of plasma TpP and D- dimer in patients with acute cerebral infarction ( ACI) . Methods D- dimer and thrombus precursor protein (TpP) levels in plasma of 55 cases with ACI during early phase and 25 normal healthy subjects were detected, and 24/55 cases were measured dynamically. D - dimer levels were detected by automated latex D- dimer immunoassay. Plasma TpP level was determined using a new assay, the TpPTM (USA), which is based on an ELISA method. Results There was significant difference ( P 〈 0.01) between the levels of D - dimer (0.98 ± 0.47mg/ml) and TpP (21.75 ± 11.69mg/ml) in patients with ACI and the control group (0.25 ± 0.17mg/ml, 2.17 ± 1.25mg/ml respectively) ( P 〈 0.01) . The longer infarction time was, the higher the D - dimer was ( P 〈 0.01) . On the contrary, the concentration of TpP was decreased gradually, especially after the effective treatment. Conclusion TpP、 D- dimer are not only reliable indicators to cases with ACI during early phase, but also can be used for monitoring the development and therapeutic efficacy of disease.
