摘要
目的探讨肥胖与内皮损伤的关系及其相关因素。方法采用化学比色法、放免法和RT-PCR法分别测定40例单纯超重和肥胖组与25例正常对照组的血浆NO、ET-1水平和单个核细胞MCP-1表达水平的变化。同时测定FPG、BMI、WC、WHC、TC、TG、HDL、LDLI、RI、HBCI等临床指标。结果单纯超重和肥胖组的NO显著降低,ET-1和MCP-1显著升高(均为P<0.000 1),逐步回归分析示BMI、TCI、RI对血浆NO有影响(标准偏回归系数分别为-0.5754-、2.960 9-、0.204 9);BMI、HDL、TCI、RI对血浆ET-1有影响(标准偏回归系数分别为0.35、-0.25、0.41、0.20);HDL、TC、WC对MCP-1表达有影响(标准偏回归系数分别为-0.03、0.96、0.03)。结论肥胖时脂代谢紊乱、IR、肥胖程度、脂肪分布异常均是引起内皮损伤的独立因素,且脂代谢中的胆固醇升高是首要因素。
Objective To investigate the relationship between obesity and endothelial damage and the related factors in the process. Methods To measure the change of NO,ET-1 and MCP-1 in the plasma of single overweight and obesity group (40 cases) and normal control group (25 cases) by RIA and RT-PCR. Results The NO level obviously decreased and ET-land MCP-1 sufficiently increased(all P〈0. 0001) in single overweight and obesity group. Stepwise regression indicated that BMI, TC, IRI had influence on NO (standard partial regression coefficient were -0. 575 4,-2. 960 9,-0. 204 9 respectively) ,BMI HDL TC and IRI had effect on ET-1( standard partial regression coefficient were 0.35, - 0. 25,0. 41,0. 20 respectively) ; HDL, TC, WC worked on the expression of MCP-1(standard partial regression coefficient were -0.03,0.96,0.03 respectively). Conclusion In obesity, the lipid decompensation,IR, the degree of obesity and distribution of fat are all independent factors attributed to cause endothelial damage, moreover the increased cholesterol is the most important factor.
