摘要
目的:观察健康查体者血压水平及所并存的其他状况来认识高血压前期的危害。方法:收集2003-06/12西安市第四医院职工1106名的健康查体资料,排除资料不全者,完成所有检查者768名列入统计,均知情同意。将所有查体者按血压水平分为4组,即正常血压组(<120/80mmHg)403名、高血压前期组(120~139/80~89mmHg)194名、1级高血压组(140~159/90~99mmHg)114名及2级高血压组(>160/100mmHg)57名。对比各组体检对象的年龄、心率、收缩压、舒张压、脉压以及在职状况、性别比例、空腹血糖、血脂和心电图异常率。测血糖采用葡萄糖氧化酶法,结果>6.1mmol/L为异常;测血脂采用酶法,以三酰甘油>1.71mmol/L和(或)血清胆固醇>6.5mmol/L为血脂异常;出现以下任何一项者为心电图异常:心动过缓、心动过速、ST-T改变、Ⅱ度Ⅰ型以上房室传导阻滞、完全左、完全右、左前束支传导阻滞及左心室肥厚等。结果:768名体检对象全部进入结果分析,无脱落。①随着血压水平的增加,各组体检对象的年龄、心率、收缩压、舒张压、脉压以及退休比例、男性比例、空腹血糖、血脂和心电图异常率亦逐渐增加(P<0.05~0.01)。②高血压前期组体检对象的年龄、心率、收缩压、舒张压及脉压均显著高于正常血压组(P<0.01)。③高血压前期组的退休人员、男性比例及心电图、空腹血糖异常率均显著高于正常血压组(P<0.05~0.01)。结论:人群中随着血压水平的升高,相应伴随年龄、脉压增大及空腹血糖、血脂和心电图异常率增加,这种情况在高血压前期即可出现。
AIM: To know the harm of earlier-stage hypertension by observing level of blood pressure (BP) and other coexisting status of people who received health checks.
METHODS: Health-check data of 1 106 employees of Xi 'an Fourth Hospital from June to December 2003 were collected, those with incomplete data were excluded. Totally 768 subjects who had fulfilled all checks were involved in the statistics and agreed with the items. Subjects were divided into 4 groups according to BP: Normal BP group (n=403, 〈 120/80 mm Hg), earlier-stage hypertension group (n=194, 120-139/80-89 mm Hg), grade 1 hypertension group (n=114, 140-159/90-99 mm Hg) and grade 2 hypertension group (n=57, 〉 160/100 mm Hg). Age, heart rate (HR), systolic pressure (SBP), diastolic pressure (DBP), pulse pressure (PP) along with working state, proportion of gender, fasting blood glucose, blood lipid and rate of abnormality in electrocardiogram (ECG) of subjects in each group were compared. Blood glucose was determinated with glucose oxidase method, those with the results above 6.1 mmol/L were taken as abnormal; Blood lipid was assayed with enzymic method and those with triacylglycerol above 1.71 mmol/L and (or) serum cholesterol (SO above 6.5 mmol/L were considered as abnormal blood lipid; ECG was thought as abnormal if any one below presented: bradycardia, pyknocardia, ST-T changing, type Ⅰ atrial ventricular block (AVB) of Ⅱ°, complete left fascicular block, complete right fascicular block, left anterior fascicular block, left ventricular hypertrophy and etc.
RESULTS: A total of 768 subjects who had received health examination were involved in the analysis and no one withdrew from the study. ① With the increasing of BP, age, HR, SBP, DBP PP, proportion of retirement, male proportion, fasting glucose, blood lipid and abnormal rate of ECG also gradually increased (P 〈 0.05-0.01). ② Age, HR, SBP, DBP and PP of subjects in the earlier-stage hypertension group were obviousl
