丹参对肝硬化门脉高压血流动力学及血管活性物质影响的相关性 被引量:13

摘要 目的研究丹参对肝硬化门脉高压患者门脉系统血流动力学及血管活性物质的相关性影响。方法49例肝硬化门脉高压患者随机分为丹参治疗组与常规对照组,采用多普勒超声在治疗前与治疗2周后检测门脉血流动力学指标的变化,同时检测了丹参治疗前后血浆内毒素、一氧化氮、内皮素、前列腺素、心钠素及肿瘤坏死因子的水平变化。分析丹参对血管活性物质与门脉压力关系的影响。结果与治疗前比较,丹参治疗后门、脾静脉内径及其血流量明显减小(P<0.05),但内毒素及血管活性物质水平降低不明显。结论丹参能有效地降低肝硬化门脉高压,但对内毒素及血管活性物质水平无明显影响。 Objective To investigate the effects of radix salviae militiorrhiza (RSM) on hemodynamics and the vasoactive substances in liver cirrhosis with portal hypertension. Methods Forty-nine cases with decompensated liver cirrhosis were enrolled and divided randomly into two groups. The control group was treated with conventional therapy and the study group was treated with RSM. Portal hemodynamics indexes were determined by Doppler before and 2 weeks after treatment in study group and control group. Plasma levels of endotoxin, nitric oxide, endothelins, prostacychn, atrial natriuretic peptide and tumor necrosis factor-or were detected, respectively. The relationship between vasoactive substances and portal hemodynamics after treatment with RSM was analyzed Results After administration of RSM, the diameter of portal vein and splenic vein and blood flow in portal vein and splenic vein were significantly decreased ( P 〈 0. 05 ). Vasoactive substances and endotoxin after treatment with RSM were not significantly decreased. Conclusion RSM can effectively decrease portal hypertension, but not affect the endotoxin and vasoactive substances.
