摘要
目的:探讨过敏性紫癜(AP)的遗传背景及免疫学发病机制,为明确AP的病因学及预后提供免疫学方面的资料,同时也为探索其免疫治疗提供有价值的线索。方法:采用分析性研究策略和聚合酶链反应——序列特异性引物分型技术(PCR-SSP)对80例内蒙古地区汉族儿童AP病人和108例正常汉族儿童进行HLA-DRB 1等位基因型别分析。结果:汉族儿童HLA-DRB 1各等位基因均被检出。HLA-DRB 1*010x、HLA-DRB 1*120x的基因频率在AP患儿组较正常对照组明显升高(分别为13.75%vs 4.16%、12.50%vs5.56%,);HLA-DRB 1*080x的基因频率在AP患儿组较正常对照组明显降低(2.50%vs 8.33%)。这些基因频率在两组间的分布经统计学检验差异有显著性。其它HLA-DRB 1等位基因频率在两组间的分布经统计学检验差异无显著性。结论:HLA-DRB 1*010x、HLA-DRB 1*120x与AP的易感性呈正相关,可能是AP的易感基因。HLA-DRB 1*080x与AP的易感性呈负相关,可能是AP的保护基因。
Objective: To investigate the correlation of the polymorphism of HLA--DRB1 alleles and Han nationality children patients with anaphylactoid purpura (AP) in Inner Mongolia in order to interpret the genetic basis of susceptibility and immune pathogenesis of AP. Methods: Eighty Han nationality children patients with AP and 108 normal Han nationality children controls in Inner Mongolia Autonomous Region were tested for HLA- DRB1 allelic types analysis by PCR--SSP technique. Results: HLA--DRB1. 010x gene frequency and HLA--DRB1 * 120x gene frequency were significantly higher in patients with AP than in the controls (13. 75% vs 4. 16%, 12.50% vs 5. 56% respectively); HLA--DRB1 * 080x gene frequency was 50% vs 8. 33%). Among AP patients signi and ficantly lower in patients with AP than in the controls(2. controls, the difference of these gene frequencies had statistical significance. There were no statistically significant differences in other alleles between the two groups. Conclusion: HLA--DRB1 * 010x and HLA--DRB1 * 120x are positively correlated with the suspeetibility of AP, which are probable the susceptible genes of AP among Han nationality children patients with anaphalactoid purpura in Inner Mongolia, and HLA--DRB1 * 080x is negatively correlated with the suspectibility of AP, which is probably the protective gene of AP.
