应用右心声学造影时间-强度曲线定量评价犬右心收缩功能 被引量:2

Quantitative assessment of right ventricular systolic function by the analysis of right ventricular contrast time-intensity curve

摘要 目的:探讨心腔声学造影时间-强度曲线(TIC)参数定量评价右心收缩功能的可靠性及寻找反映右心收缩功能的可靠指标。方法:10只犬分别在基础状态及注射心得安后于心功能减退状态下经股静脉重复注射5%声振清蛋白注射液,剂量0.08ml/kg,选取心尖四腔切面观察右心室造影剂的充盈及清除。采用拟合曲线定量分析方法获取心腔声学造影TIC参数,并分析参数在不同的心功能水平的差异。结果:随心功能的减退,时间常数(k)明显减小,而造影剂峰值强度减半时间(HT)及造影剂平均通过时间(MTT)明显延长(P<0·01)。k、HT及MTT分别与右心排血量、左室射血分数左室短轴缩短率明显相关。结论:采用拟合曲线定量分析技术获得右心腔声学造影TIC,用于定量估测右心收缩功能是可靠的,其中k、HT和MTT是反映右心收缩功能的可靠指标。 Objective: To study on reliability and reliable indexes of quantitative assessment of right ventricular systolic function by time-intensity curve (TIC) with right ventricular contrast. Method: The 5% sonieated human albumin was injected intravenously in does of 0.08 ml/kg to baseline and deereasd condition of cardiac function after injected propranolol in 10 dogs. Four chambers view was observed for wash in and wash out of contrast agent from right ventricle. The parameters of TIC were obtained by curve fitting. The differents of parameters was analyzed in different cardiac function. Result: Among the parameters derived from TIC, the time constant(k) was decreased significantly with decrease of cardiac function (P〈0.01). But half-time of decent of peak intensity and mean-transit-time (MTT) of wash out were increased significantly (P 〈0.01). k, half-time of decent of peak intensity (HT) and MTT was respectively strongly related with output of right ventricle, ejection fraction of left ventricle (LVEF), fractional shortening of left ventricle. Conclusion: Right ventricular systolic function measured quantitatively by the parameters derive from TIC with right ventrieular contrast was reliable, k and HT, MTT are the reliable indexes of quantitative asssment of right ventricular systolic function.
