摘要
目的探讨后凸成形术疗效评估中骨折椎体高度的测量方法。方法选8套干燥成人胸腰椎标本,按解剖序列拍摄X线侧位片。分别测量T1~L5标本及X线片上椎体前缘、中央、后缘高度;以椎序为自变量、椎体高度为应变量进行直线回归与相关分析,探讨压缩骨折椎体原有高度的预测方法。结果T1~L5椎体前缘、中央、后缘高度,无论是标本测量还是影像学测量,均与椎序分别成直线关系(P=0.0001。r〉0.9);各高度影像学测量与解剖学测量值之间一致相关(r〉0.9)而不受X线投照中心的影响;压缩椎体骨折前的高度可以接直线方程推出的公式进行推算。结论在同一X线片上选择两个恰当参照椎。可按公式准确求得压缩椎体骨折前的高度,为后凸成形术疗效评估中骨折椎体高度的测量提供准确可靠的方法。
Objective To discuss the method of the vertebral height measurement before fracture in the the outcome evaluation of kyphoplasty for osteoporotic vertebral compression fractures. Methods The anterior, mid and posterior vertebral heights were measured in 8 spine specimens and in their images. It was analyzed that the linear regression and correlation between the vertebral level as an independent variable and the vertebra heights in specimen and images and the formulation to measure the vertebral height before fracture. Results There is a linear relationship between the height from T1 to L5 in specimen and images and there levels were discussed (P = 0. 0001, r 〉0.9) ; the vertebral height of image is related with anatomy but it is not affected by the center of X ray ( r 〉 0.9) and the height before fracture can be calculated with the formulation from the linear equation. Conclusion The vet tebral height before fracture can be exactly obtained by choosing two fit reference vertebrae according to the formulation.
