摘要
目的 探讨椎体成形术椎体形态改变与疗效的关系。方法 本组 36例 4 7个椎体 ,按椎体骨折部位的不同分为三个类型 :椎体上终板骨折塌陷型、椎体中央骨小梁断裂塌陷型、椎体下终板骨折塌陷型。通过X线片观察 ,测量并计算三种类型术前、术后椎体前缘和后缘的高度及比值的变化 ,同时观察术后疗效。结果 不同类型的椎体骨折术前、术后计算出的比值不同 ,显示骨折椎体前缘高度恢复情况不同。本组 36例均获随访 ,时间 8～ 15个月 ,平均 12 5个月。结论 不同类型的椎体骨折在脊柱正常伸屈活动时存在着不同的运动形式 ,手术通过复位骨水泥填充 ,最大限度阻止骨折椎体的运动是术后疗效的保证。
Objective To study the morphological change vertebral body after vertebroplasty and observe the their outcome.Methods There were 36 cases (total 47 vertebrates) in this group. The vertebral body fractures were diveded into three types: ① upper end-plate depressed fracture;②vertebral body central trabecula fracture;③inferior end-plate depressed fracture. All patients had a x-ray check soon after operation, measured the height of anterior edge (AE) and posterior edge (PE) (post-operation compare to pre-operation), and calculated AE/PE ratio, studied the relationship of the AE/PE change of the postoperation and the treatment outcome.Results The AE/PE ratio and the anterior edge recovery was different in the three types fracture. All of 36 cases in this group were followed-up for 8～15 months with average 12.50 months.Conclusion To measure and calculate AE/PE ratio of pre-operation and post-operation, it can indicate the morphological change authentically and scientifically.There is a different movement formation in different type of fracture when the spine fling and extending,the treatment outcome is related to the vertebral body anterior edge recovery after operation.
