翼状胬肉切除术中与术后应用丝裂霉素C的比较 被引量:10

Comparison of intraoperative MMC and postoperative MMC in preventing recurrence of pterygium

摘要 目的丝裂霉素C能抑制翼状胬肉切除术后的复发,对胬肉切除术术中应用丝裂霉素C与术后应用的结果进行比较,评估两种方法的有效性和安全性。材料与方法将原发性翼状胬肉患者随机分为两组,术中应用0.02%丝裂霉素C组和术后用0.02%丝裂霉素C组,观察两组患者术后复发率和并发症情况。结果共有91例患者103只眼纳入本研究,其中术中用丝裂霉素C组50只眼中3眼复发(6.0%),术后用丝裂霉素C组53只眼中4眼复发(7.5%),两组间差别无统计学意义(p>0.05);随访期间两组患者均没有出现严重的并发症。结论翼状胬肉切除术联合术中应用丝裂霉素C与术后应用丝裂霉素C都是治疗翼状胬肉有效的方法,而术中应用丝裂霉素C可能更为安全。 Aim mitomycin C (MMC) is a useful adjuvant in the prevention of pterygial recurrence. This study was conducted to compare the outcome of intraoperative MMC and postoperative MMC in preventing recurrence of pterygium. Methods Prospective study on consecutive cases of primary pterygium randomized into two adjuvant groups:(1) intraoperative 0.02% MMC for 5 minutes or (2) postoperative 0.02% MMC for 5 days. Patients were followed for recurrence and complications for a period of six months. Result 103 eyes in 91 patients who completed the study were randomized to receive intraoperative MMC (n = 50) and postoperative MMC (n = 53). There were 3 recurrences (6.0%) in the intraoperative group and 4 recurrences (7.5%) in the postoperative group. There was no statistically significant difference in the recurrence rate between the two groups (p 〉0.05). No significant complications were encountered in either group. Conclusion Simple excision combined with intraoperative MMC or postoperative MMC are both effective adjuvants for pterygium excision, but intraoperative MMC may be safer. Because of the possibility of serious late complications, controlled use of MMC and long term follow up are required.
