摘要
目的:探讨人工全髋关节置换术中对下肢严重短缩的延长和平衡处理。方法:选择患肢短缩>4 cm的患者,在软组织松解的基础上,维持肢体的延长、假体的稳定性以及髋部肌力之间的平衡。术后评估肢体长度及髋关节功能。结果:手术后平均延长肢体长度为4.7 cm,Harris评分平均提高39.4分。结论:通过术中重建髋臼、适当松解或截骨以及选择合适的假体,能较好地解决患肢严重短缩现象,获得下肢肢体的相对平衡。
Objective : To discuss the methods of leg-length balance and extend for patients with serious shorten undergoing total hip arthroplasty. Methods: Twenty-two patients with leg-length shorten more than 4 cm were reviewed. On the basis of widely release with soft tissue, the balance between the extending of leg-length, the prosthesis stability and the muscle strength around hip were considerated. The leglength and Harris scores were evaluated. Results : The leg-length was extended 4.7 cm and the Harris scores increased 39.4. Conclusion: The serious shorten of leg-length could be resolved preferably through the acetabular reconstruction, the proper release of soft tissue, and the selection of suitably prosthesis. The leg-length may achieve a relatively balance.
