兔耳增生性瘢痕形成与外科常见致病真菌感染的关系 被引量:2

摘要 目的:分析外科感染常见致病真菌对兔耳创面增生性瘢痕组织块形成的影响。方法:实验于2004-02/07在新疆医科大学动物实验室和新疆医科大学病理科完成。选取新西兰大耳白兔12只,随机分为白色念珠球菌污染组、金黄色葡萄球菌污染组、空白对照组,4只/组。在各组兔耳腹侧制作直径为1cm全层皮肤缺损创面,创面形成时及术后第1天白色念珠球菌污染组分别于创面放置微量白色念珠球菌,金黄色葡萄球菌污染组于创面放置微量金黄色葡萄球菌,空白对照组不予任何干扰因素。观察各组创面愈合及增生性组织块发生情况,测定各组术后不同时间点成纤维细胞数及转化生长因子β1和胶原Ⅰ的表达。结果:实验纳入12只兔,兔耳创面制作时因麻醉药物过量4只死亡,予以补充。①白色念珠球菌污染组和金黄色葡萄球菌污染组兔耳腹侧创面愈合后可出现类似人增生性瘢痕的增生组织块,愈合时间均长于空白对照组[(14.7±1.0),(15.3±1.1),(11.2±1.3)d,P<0.01];增生组织块出现率均高于空白对照组(85.4%,91.6%,62.5%,P<0.05);术后28,49,60d增生组织块成纤维细胞含量均明显高于空白对照组(P<0.01)。②各组成纤维细胞中转化生长因子β1及胶原Ⅰ在术后28,49d均为强阳性表达,白色念珠球菌污染组最为显著,随时间的推移各组表达逐渐减弱直至消失。结论:白色念珠球菌和金黄色葡萄球菌污染创面均易出现增生性组织块,其形成与创面愈合过程关系密切,创面愈合时间越长增生组织块的出现率越高,符合人增生性瘢痕变化规律,验证转化生长因子、成纤维细胞及胶原之间的密切关系构成了增生性瘢痕的生物学基础。 AIM: To analyze the effects of fungus of surgical infection on the formation of hypertrophic scar in rabbits ears after injury. METHODS: The experiment was conducted at the animal laboratory and Department of Pathology of Xinjiang Medical University from February to July 2004. Twelve New Zealand white rabbits with big ear were selected, and randomly assigned into candida albicans group, staphylococcus aureus group and blank control group with 4 rabbits in each group. Full-thickness excisional wounds and lcm in diameter were created over the ventral surface in every group. Small amount of candida albicans and staphylococcus aureus were laid on the wound of rabbits in the candida albicans group and staphylococcus aureus group when the wound formed and at the 1st day after operation, respectively. Those in the blank control group were not intervened. The wound healing of the three groups and incidence of forms of excessive scar were observed. The number of fibroblasts, expression of transforming growth factor beta 1 and collagen x were investigated in different time. RESULTS: Among the 12 included rabbits, 4 died in wound creation of rabbit ears because of drug overdose, and complement was done. ① Excessive dermal scars on rabbits ears that were similar to human hypertrophic scar appeared in the candida albicans group and staphylococcus aureus group.The wound healing time of the three groups was longer than that in the blank control group [(14.7±1.0), (15.3±1.1), (11.2±1.3) d,P 〈 0.01]. The presenting rate of excessive dermal scars was all higher than that in the blank control group (85.4% ,91.6% ,62.5% ,P 〈 0.05). After operation for 28, 49 and 60 days, contents of fibroblasts of excessive dermal scars were all higher than those in the blank control group (P 〈 0.01). ② The transforming growth factor beta 1 and collagen Ⅰ of the fibroblasts in every group showed positive expression after operation for 28 and 49 days.
