颞下颌关节紊乱病关节镜检查与关节内压相关性分析 被引量:1

摘要 目的了解颞下颌关节紊乱病患者关节镜检查与关节上腔中压力变化规律的关系,初步探讨颞下颌关节上腔不同病变类型对其生物力学性能的影响。方法收集颞下颌关节紊乱病患者16例22侧,用关节内压测量仪通过穿刺针进入关节上腔,分别测量患者张闭口位的关节上腔内压,并记录波形和平均值,测压后进行颞下颌关节镜检查。结果将患关节分为低压型、反压型和高压型。低压型关节和反压型关节的镜下表现差异有显著性(P=0.002<0.01);反压型与高压型关节的镜下表现差异有显著性(P=0.003<0.01);而低压型与高压型病例差异无显著性(P=0.045>0.01)。结论关节腔内部的粘连程度和类型的不同影响着关节腔内压的变化规律,关节内压分类法能部分体现关节腔内的病理状况。 [Objective] The study aims to find out relationship between of arthroscopy and the intra-articular pressure in temporomandibular joint with anterior disc displacement without reduction. It also discusses its influence to the joint's biomechanical behavior. [Methods] It included the collection of 22 affected sides in 16 patients. We used a No.8 syringe needle to pierce into the upper compartment of TMJ. The pre-arthroscopy intra-articular pressure was taken by the pressure transducer via the flexural rigid tubing at open and close bite. The curve and mean value were documented. [Results] The pressure values were classified into 3 types: high pressure, reverse pressure and low pressure. The difference of arthroscopy views between the first and second types was statistically significant (P =0.002〈0.01). The pressures when the second and third types showed statistical significant difference (P =0.003〈0.01), The difference of arthroscopy views between the first and third types was not statistically significant (P =0.045〉0.01). [Conclusions] The adhensive degree affects the fluctuating curve of the TMJs with anterior disc displacement without reduction. The classification based on the intra-articular pressure can partly reflect the pathology condition of the joint.
