语前聋患者人工耳蜗术前评估及其意义 被引量:2

摘要 目的:探讨人工耳蜗植入术前评估对语前聋儿童治疗的重要性。方法:回顾30例在我院接受人 工耳蜗植入、开机至少6个月的语前聋患者的术前检查资料,根据人工耳蜗植入工作指南,结合术后听觉康 复疗效进行分析。对全部患者均采用回顾临床资料和进行自由声场测试纯音、言语察觉率和开放式、闭合 式言语分辨率的方法及噪音中语言辨别率的测定及其电话测听,评价人工耳蜗补偿听力的效果。结果:23 例已上正常幼儿园及小学,全部患者声场测听均低于40 dB,言语均明显提高,3例能接听电话。结论:颞 骨高分辨率 CT、MRI 耳蜗水成像等影像学检查;纯音测听和听觉诱发电位(ABR)、声导抗、耳声发射(DPOAE)、 40Hz 相关电位(40HzAERP)40HzAERP 和耳镜检查对手术的成功性,预防并发症以及患者听觉康复的可能疗 效有非常重要的意义,术中 NRT、电阻检测评估手术的成功性及术后程序编程处理有一定的参考意义。 Objective: To assess the significance of preoperative evaluation in prelingually deaf children with cochlear implant Methods: Retrospective analyses with preoperative tests in 30 children with cochlear implant. According to the cochlear implant guide (2003, changsha) and the postoperative analyses with effect of auditory rehabilitation. We evaluate the effect of auditory rehabilitation through clinical data, pure tone sound field audiometry, speech detection threshold, opened-closed speech separation rate, language discrimination factor and telephone audiometry. Results: Among all the cases, 23 has studied in kindergartens or Elementary schools normally, 3 could answer telephones, the pure tone sound field audiometry were all lower than 40dB, speech ability were all improved obviously. Conclusions: The preoperative measures, including imaging examination, auditory analysis and otoscopy were significant to the success of operation, the avoidance of complications and the effect of auditory rehabilitations. There is definite reference significance in evaluation of the success of the NRT and the electric impedance test during operation and in postoperotive program.
