摘要
目的研究颌面部爆炸伤时心脑并发伤的特点与致伤机制。方法10只犬按伤后0、6、24h时及对照组随机分组,对犬左咬肌区利用高速破片+冲击波致伤模型模拟软组织爆炸伤,观察动物伤后0、6、24h心脑损伤情况。结果伤后6h脑损伤以出血及粒细胞浸润为主,伤后24h损伤加重;2只犬心脏出现内膜下出血及内皮细胞脱落。结论颌面部爆炸伤时多种因素的复合作用可导致心脑并发伤,抢救时须综合考虑,以防止发生多器官功能衰竭。
Objective To study the characteristics and mechanism of combined injuries of hearts and brains after explosive injuries of maxillofacial regions. Methods The models of explosive injury in maxillofacial region were induced by blast wave and high-velocity projectile in 10 dogs. Local tissues were collected and specimens of the hearts and brains were obtained at 0, 6, and 24 hours after explosion, which were used for histological investigation by HE staining. Results Hemorrhage and neutrophil infiltration were observed in brains at 6 hours after injury, and aggravated at 24 hours. Two dogs were found with sub-endocardium hemorrhage and endothelia falling-off in the heart. Conclusion Explosive injury in maxillofacial region can cause combined injuries of the heart and the brain. When treating maxillofacial explosive injury, careful examination should be made in order to avoid multiple organ failure (MOF).
