摘要
目的分析急性脑梗死患者血浆CD62p和IL-6水平动态变化及临床意义,探讨其在脑梗死后炎性病理损伤中的可能作用机制。方法用ELISA法和放射免疫法测定60例脑梗死患者发病第48h,第6 ̄8天及第15天血浆CD62p和IL-6水平,同期测定对照组为30例年龄,性别相匹配的健康者血浆CD62p和IL-6水平。所有脑梗死患者依据发病后第48~72h的CT检查结果计算脑梗死体积。结果发病48h内脑梗死患者血浆CD62p和IL-6浓度均显著高于正常对照组(P<0.01);血浆CD62p和IL-6水平随发病后时间的推移而逐渐下降,血浆CD62p水平在发病第2天[(9.25±1.82)ng/mL],第6 ̄8天[(7.12±1.75)ng/mL]和第15天[(3.55±1.43)ng/mL]三个时间点两两比较差异显著(P<0.05);IL-6水平在发病第2天[(365.4±28.8)pg/mL]与第6 ̄8天[(346.7±28.9)pg/mL]比较无统计学意义(P>0.05),发病第15天[(308.5±26.9)pg/mL]较第6 ̄8天明显降低差异有显著性(P<0.01),血浆CD62p和IL-6水平至第15天时仍高于对照组(P<0.01);发病6 ̄8d内血浆CD62p和IL-6水平均和脑梗死体积呈正相关(r=0.368,P<0.05和r=0.594,P<0.01),血浆CD62p和IL-6水平呈正相关(r=0.453,P<0.05)。结论急性脑梗死患者血浆CD62p和IL-6升高,二者可能参与了脑缺血炎性病理损伤,CD62p的升高可能与IL-6升高有关,二者水平可能能够反映脑缺血后炎性病理损伤的程度和脑梗死体积大小。
[Objective] To analyze the change and clinical significance of plasma CD62p, IL-6 in the patients with cerebral infarction, and to explore the pathologic mechanism of the brain ischemia. [Methods] The levels of Plasma CD62p, IL-6 were measured by ELISA and radioimmunoassy 60 patients at 48 hours, 6-8 days and 15 days after ischemia. The levels of plasma CD62P, IL-6 of 30 healthy person mating cerebral infarction group in sex and ages were measured. The subsequent volume of brain lesion as a consequence of stroke was measured by CT within 48-72 hours after the onset. [Results] The levels of plasma CD62P, IL-6 in the patients with acute cerebra/infarction were obvious higher than the levels of the healthy control groups( P 〈0.001); The levels of plasma CD62p, IL-6 declined gradually with the time of the onset. The level of Plasma CD62p at 48 hours [(9.25±1.82) ng,/mL] was obvi- ous higher than the levels 6-8 days [(7.12± 1.75) ng/mL] and 15 days [( 3.55±1.43)ng/mL], the differences were obvious (P〈0.01); The levels of plasma CD62P, IL-6 in the patients with acute cerebral infarction at 6-8 day were also higher than the levels of the healthy control groups ( P〈0.001); The levels of plasma CD62P,IL-6 at 6-8 days after the onset were positive correlation with the infarct volume (r =0.368, P〈0.05 and r =0.594, P〈0.01); There was a positive correlation between the levels of plasma CD6zp and the levels of plasma IL-6. [Conclusion] The levels of plasma CD62p, IL-6 were upregulated, which might play a role in inflammatory lesion, and the upregulafion of CDazp may be related to the IL-6, The levels of plasma CD62P, IL-6 may reflect both the degree of pathologic lesion after the brain ischemia and the infarct volume.
