过氧化物酶体增殖物激活受体γ2基因Pro-12Ala多态性与代谢综合征的相关性研究 被引量:5

The influence of the Pro12Ala mutation of PPARγ2 receptor gene clinical character in metabolic syndrome

摘要 目的旨在探讨过氧化物酶体增殖物激活受体γ2(PPARγ2)基因Pro12A la多态性对β细胞功能储备、胰岛素抵抗作用及与代谢综合征的相关性。方法随机选取湖北地区无亲缘关系的新诊断的2型糖尿病汉族人289例,其中伴高血压病组(HTD)132例,血压正常组(NTD)157例。应用聚合酶链反应-限制性片段长度多态性技术,进行基因突变检测。结果(1)湖北地区汉族人群体中2型糖尿病A la12携带者(和A la/A la基因型)及A la12等位基因频率分别为0.042;0.015,与正常人群没有差异;本研究中PPARγ2 Pro12A la与2型糖尿病患者性别、年龄、体重指数、血脂谱无相关性。(2)PPARγ2Pro12A la多态性与2型糖尿病的高血压呈负相关。(3)在HTD组中PPARγ2 Pro12A la与保护β细胞对葡萄糖刺激胰岛素分泌反应(P<0.05)、减少胰岛素抵抗有关(P<0.05)。结论PPARγ2基因是2型糖尿病、高血压病的重要遗传因素,PPARγ2基因负性突变是2型糖尿病和胰岛素抵抗相关疾病的保护因素。 Objective The aim of this investigation was to determine whether a PPAR γ2 Pro12Ala polymorphism was associated with clinical character in metabolic syndrome. Methods 289 unrelated Chinese subjects first diagnosed Type 2 diabetes (HbAlc 〈 6.0% ) were investigated, including 132 hypertensive diabetic (HTD) subjects, 157 normotensive diabetic (NTD) subjects. Blood pressure and anthropometric measurements were collected from all participants, as well as several venous blood samples during OGTT. Biochemical measurements ( high-density lipoprotein (HDL) and low-density lipoprotein- cholesterol(LDL) ,triglycerides) and PPARγ2 Prol2Ala genotype were also determined. And insulin resistance and β-cells function was assessed by HOMA-IR and HOMA-β respectively. Results The frequency of subjects bearing the Pro12Ala was lower in the hypertension group (3.03%) than in the nonhypertension group ( 5.7% ) ( P 〈 0. 05 ) after adjusted for age, BMI, gender. Hypertensive diabetic Pro12Ala subjects had lower fasting plasma glucose level (P 〈 0. 05 ) , and better glucose tolerance at 60 min after oral glucose ( P 〈 0.05 ) ; simultaneously, HOMA-β% , AUC for insulin and HOMA-IR got the same result. Conclusions Our findings suggested that the Ala 12 allele of the PPAR γ2 gene may improve insulin resistance and ameliorate β-cell function restoration in T2DM with hypertension, and protect from hypertension in T2DM. As an important thrifty gene, environment factors may make a role of the effect of PPAR γ2 on glucose homeostasis and insulin resistance.
